Objective: To compare the clinical effect of long head tendon disconnection of biceps brachii with rotator cuff injury in order to provide more choices for clinical treatment.Methods: From October 2018 to April 2020 in the Affiliated Hospital of Chengdu University of traditional Chinese medicine,60 patients(all unilateral shoulder joint),18 males,who were diagnosed with biceps tendon disorder and rotator cuff injury were selected,there were 42 females aged between 44 and 58 years with an average age of 55.33 years.All the subjects were examined by positive plain film of the shoulder joint,superior supraspinatus exit(y)plain film and magnetic resonance imaging(MRI)of the shoulder joint.According to the different treatment of long head tendon of biceps brachii during shoulder arthroscopy,30 cases were divided into two groups(tenotomy group)and 30 cases(tenotomy group).General medical records,operative time and upper arm circumference before and after surgery were collected,v AS scores,UCLA scores and Constant-murley function scores of shoulder joint were analyzed before operation,1 week after operation,2 weeks after operation,1 month after operation,3 months after operation,6 months after operation and 1 year after operation.Results: 1.Complete follow-up was performed in 60 patients with tenotomy and tendon preservation.2.There was no significant difference in the general data between the tenotomy group and the tendon preservation group(P > 0.05).The time of operation in the group of tendon resection was significantly shorter than that in the group of tendon preservation(p < 0.05).4.There was no significant difference in arm circumference between the tenotomy group and the tendon preservation group before and after operation(P > 0.05).5.There were significant differences in preoperative VAS score,UCLA score and Constant-murley function score of the shoulder joint between all the subjects(p < 0.05).VAS scores,UCLA scores and Constant-murley function scores of shoulder joint in the tenotomy group were significantly higher than those in the tenotomy group(p <0.05).VAS score,UCLA score and Constant-murley function score of shoulder joint in 6 months and 1 year after tenotomy were not significantly different from those in tenotomy group(P > 0.05)。Conclusion: Arthroscopic disconnection of the long head of the biceps brachii has obvious advantages in early postoperative pain and functional rehabilitation compared with tendon preservation,but in the medium and long term,both of them are effective methods for the treatment of long head tendon disorder of biceps brachii combined with rotator cuff injury.
