Clinical Observation Of Duhuo Jisheng Decoction Combined With Percutaneous Endoscopic Lumbar Discetomy In The Treatment Of Cold And Wet Lumbar Disc Herniation

Purpose:To observe the clinical efficacy of Duhuo Jisheng Decoction combined with percutaneous endoscopic lumbar discectomy for the treatment of cold dampness type lumbar disc herniation.Methods:The research subjects were 60 patients diagnosed with lumbar disc herniation(L4/5 or L5/S1)who were hospitalized in the Department of Orthopedics and Traumatology,Affiliated People’s Hospital of Fujian University of Traditional Chinese Medicine from March 2022 to December 2022.The patients were randomly divided into a control group and an experimental group,with 30 cases in each group.The control group was treated with percutaneous endoscopic lumbar discectomy,mecobalamin tablets,and celecoxib capsules,while the experimental group was treated with Duhuo Jisheng Decoction in addition to the control group,with a course of 4 weeks.Observe and record the VAS score,JOA score,traditional Chinese medicine syndrome score,and post-treatment efficacy of two groups of patients before and 1 week,1 month,and 3 months after treatment.Measure the C-reactive protein level and safety observation indicators before and 1 day,1 week,and 1 month after treatment to observe their clinical efficacy and safety.SPSS 23.0 was used for statistical analysis of the obtained data.Results:1.A total of 60 patients were included in this study,with 2 cases falling off.The final control group consisted of 29 patients and the experimental group consisted of 29 patients.The general situation of the two groups of cases before treatment was compared,and the difference was not statistically significant(P>0.05),indicating comparability.2.VAS score: Both groups of patients showed a statistically significant decrease in VAS score after treatment compared to before treatment(P<0.05);After 1 week and 1 month of treatment,the VAS score of the experimental group was better than that of the control group(P<0.05);After 3 months of treatment,there was no statistically significant difference between the two groups(P>0.05).3.JOA score: After treatment,the JOA score of both groups of patients improved compared to before treatment,and the difference was statistically significant(P<0.05);After1 week of treatment,there was no statistically significant difference between the two groups(P>0.05).The JOA score of the experimental group was better than that of the control group at 1 month and 3 months after treatment(P<0.05).4.C-reactive protein: On the first day after surgery,the C-reactive protein levels in both groups of patients increased compared to before surgery(P<0.05).As treatment time progressed,the C-reactive protein levels in both groups decreased(P<0.05),and the experimental group was better than the control group(P<0.05).5.Traditional Chinese Medicine Syndrome Score: Both groups of patients improved their TCM syndrome scores after treatment compared to before,and the improvement effect of the experimental group was better than that of the control group(P<0.05).6.Traditional Chinese Medicine Syndrome Efficacy: The total effective rate in the control group was 86.2%,while the total effective rate in the experimental group was 93.1%.The experimental group was superior to the control group(P<0.05).7.Safety observation indicators: During the treatment period,two groups of patients underwent blood routine tests and liver and kidney function tests before and 1 week and 1month after treatment,and no significant abnormalities were found.Conclusions:Duhuo Jisheng Decoction combined with percutaneous endoscopic lumbar discectomy can significantly alleviate the symptoms of low back pain and lower limb radiation pain in patients with cold dampness type lumbar disc herniation,improve lumbar function,and reduce postoperative C-reactive protein levels.There is a clinical therapeutic effect.
