Objective: To explore the difference of ln LAEI in patients with atrial fibrillation(AF)and with heart failure with preserved ejection fraction combined with AF(HFp EF-AF).Methods: A total of 52 AF patients with preserved left ventricular ejection fraction(LVEF)who underwent transthoracic two-dimensional echocardiography in our hospital from September 1,2021 to August 31,2022 were included in this study.According to HFA-PEFF score,they were divided into group a(score <1,n=16),group b(score 2-4,n=16)and group c(score ≥5,n=20).Philips i E33 color Doppler ultrasound diagnostic apparatus was used to collect images of the selected patients.LVEF,mitral valve early and late diastolic peak flow velocity ratio(E/A),early mitral annular velocity(e ’),ratio of early peak mitral valve flow velocity to early mitral annular velocity(E/e’).The maximum volume of left atrium(LAVmax),minimum volume of left atrium(LAVmin),left atrial ejection fraction(LAEF),left atrial strain reserve phase(LASr)were analyzed by Qlab13.0 workstation.Finally,left atrial expansion index(LAEI)and its log value ln LAEI were calculated.Compare the differences between the groups.The differences between different score groups were compared.Results: Compared with group b,the E / e’,LAVmax and LAVmin of group a were decreased,LAEF、LAEI、ln LAEI、LASr increased with statistically significant differences(P< 0.05).Compared with group c,the E / e’,LAVmax and LAVmin of group a were decreased,e’、LAEF、LAEI、ln LAEI、LASr increased with statistically significant differences(P< 0.05).Compared with group c,the LAVmax、LAVmin of group b were decreased,e’、LAEF、LAEI、ln LAEI、LASr increased with statistically significant differences(P< 0.05).Conclusion: In patients with AF complicated by HFp EF,although LVEF remained in the normal range,left atrial function was prone to significant change,LAEF,LAEI,ln LAEI,LASr decreased and LAVmax,LAVmin increased.Ln LAEI is a relevant factor for the development of HFPEF and its value will further decrease with the aggravation of HFPEF,suggesting impaired LV diastolic function.Ln LAEI is a good indicator for evaluating HFPEF-AF patients.Ln LAEI has a good correlation with LASr. |
