Self-reported Treatment Goals Of Inflammatory Bowel Disease Patients In Liaoning Province During The Covid-19 Epidemic

Background: Inflammatory bowel disease is a chronic recurrent disease,and the treatment goals of previous inflammatory bowel disease are mainly based on the doctor’s perspective,but the treatment goals of inflammatory bowel disease from the patient’s perspective are lacking.IBD patients who are unable to receive systematic treatment and medication in hospitals due to personal and social factors,and due to the COVID-19pandemic(02 February 2020 to 30 April 2020).Therefore,the purpose of this study is to understand the self-reported treatment goals and associated influencing factors during the COVID-19 pandemic(02 February 2020 to 30 April 2020).Methods: An online questionnaire was conducted for patients diagnosed with inflammatory bowel disease in the Gastroenterology Department of the First Affiliated Hospital of China Medical University during the COVID-19 pandemic(February 2,2020 to April 30,2020).The questionnaire included general data,clinical characteristics,and self-reported treatment objectives.Self-report treatment objectives included 11 items:improvement of quality of life,reduction of medication,avoidance of surgery,recovery from radiographic imaging,reduction of psychological impact,total oral medication,reduced side effects,mucosal healing,normalization of biochemical indicators,relief of abdominal pain,and normal defecation.By assigning each treatment target(0-10 points),higher scores represent higher attention.Univariate and multivariate Logistic regression analyses were used to explore the influencing factors associated with these self-reported treatment goals.Results: 212 IBD patients were included in this study,and 111 males were 52.36%,with a mean age of 43 ± 0.94 years),the disease duration ranged from 1 month to 312 months.There were 49(23.11%)of Crohn’s disease(CD),145(68.4%)of ulcerative colitis(UC)and 18(8.49%)of unclassified patients.Through the self-reported treatment target score,the improvement of quality of life(mean score was 8.54),and the mean score of ulcerative colitis and Crohn’s disease was 9.10 and 8.45,respectively.In the univariate analysis,the duration of the disease was considered as(P = 0.038),number of defecation(P = 0.004),and frequency of hematochezia(P = 0.047),avoided the surgery of IBD(P =0.015)and imaging recovery(P =0.008).Reducing psychological impact is the goal of treatment is the course of the disease(P =0.041),and the associated factor is the number of defecation.Treatment objectives of total oral medication were disease course(P =0.001),disease course(P = 0.033)and frequency of hematochezia(P = 0.003),frequency of normal defecation,disease course(P =0.016),and frequency of hematochezia(P =0.048).In multivariate analysis suggested that the factors few drugs associated with defecation(P =0.01);the duration of the course of disease(P =0.033)and the frequency of hematochezia(P =0.003);the disease type associated with recovery of defecation as treatment target(P =0.001),and the frequency of hematochezia(P =0.015).Conclusion: During the COVID-19 pandemic(02 February 2020 to 30 April 2020),patients were not able to attend for treatment and assessment,so the COVID-19 outbreak could influence the self-reported treatment goals of patients with inflammatory bowel disease.Our results found that patients with inflammatory bowel disease were most concerned about the improvement of quality of life,the reduction of drugs and the avoidance of surgery;there were some related factors for these self-reported treatment objectives,specifically: the number of defecation;the duration of disease and frequency of hematochezia;and the return to normal of defecation: disease type and frequency of hematochezia.Our results help clinicians understand the patient treatment goals,which contribute to better management of patients with inflammatory bowel disease.
