| Objective:To understand the current status of medication beliefs of breast cancer patients on endocrine therapy and to analyze the correlation between adverse effects,hope,drug beliefs and quality of doctor-patient communication with medication beliefs of breast cancer patients on endocrine therapy.To identify the impact factors on the beliefs of breast cancer patients in taking medication for endocrine therapy and to provide a basis for clinical nurses to raise patients’ beliefs in their medication.Methods:In this cross-sectional study,408 patients with breast cancer who received endocrine drug therapy after surgery in a tertiary Class A hospital in Shenyang from November 2021 to November 2022 were selected by convenience sampling method.Data were collected by questionnaires,and the survey instruments included the self designed generic information questionnaire,Belief about medication Questionnaire,Herth Hope Index and Physician-patient communication quality Scale.Using Excel software for data entry,and SPSS 25.0 software was used for descriptive analysis using means,standard deviations,and composition ratios,t-test and ANOVA were used for one-way analysis,Pearson correlation analysis and multiple linear stepwise regression analysis were used to determine the influencing factors of medication beliefs,P<0.05 was considered a statistically significant difference.Results:1.A total of the study distributed 410 questionnaires,and 408 valid questionnaires was obtained,with a effective recovery rate of 99.5(4),all of which are female.The subjects ranged in age from 30 to 74,and the majority of patients ranged in age from 51 to 60(37.0(4));married patients accounted for 87.3(4)of the total;the number of people with secondary or high school education was 184(45.0(4));the percentage of patients with an average monthly household income of less than $5,000 was 55.6(4);74.2% of the patients whose medical payment method was medical insurance;the number of active patients was157(38.5(4));76.7(4)of patients who had received radiotherapy or chemotherapy;the number of patients took tamoxifen was 159(39.0(4)).2.Breast cancer patients’ beliefs about medication scored(5.31±4.10),with the highest score of 21 and the lowest score of ﹣8;Drug necessity dimension score(16.80±3.64),drug concern dimension score(11.49±2.58),drug toxicity dimension(12.16±2.15),drug overuse dimension(8.66±1.87),drug long-term effect dimension(6.88±1.76).3.The results of the univariate analysis of general data showed statistically significant differences(P<0.05)in medication beliefs among patients with different mean monthly household income,medical payment method,education level and course of endocrine therapy already received.4.Correlation analysis showed that the total score of adverse effects was negatively correlated with the belief of medication for endocrine therapy in breast cancer patients(correlation coefficient r=﹣0.134,P<0.01);the score of drug toxicity dimension was negatively correlated with the belief of medication(correlation coefficient r=﹣0.411,P<0.01);the score of drug overuse dimension was negatively correlated with the belief of medication was negatively correlated with medication taking beliefs(correlation coefficient r=﹣0.344,P<0.01);total hope level score was positively correlated with medication beliefs(correlation coefficient r = 0.267,P<0.01);total doctor-patient communication quality score was positively correlated with medication beliefs(correlation coefficient r=0.314,P<0.01).5.The results of multiple linear regression analysis showed that a total of six independent variables,drug toxicity beliefs,quality of doctor-patient communication,course of endocrine therapy received,hope,average monthly household income,and drug overuse beliefs,entered the regression equation and explained 32.1(4)of the total variance in drug beliefs about endocrine therapy in breast cancer patients,which were the main influencing factors of drug beliefs about endocrine therapy in breast cancer patients.Conclusion:1.The belief in endocrine therapy drugs for breast cancer patients is at an intermediate level and still needs further improvement.2.Adverse effects of endocrine therapy,drug toxicity dimension and drug overuse dimension were significantly and negatively associated with beliefs about medications in breast cancer patients;hope and quality of doctor-patient communication were significantly and positively associated with beliefs about medications.3.Factors influencing medication beliefs about endocrine therapy in breast cancer patients included the drug toxicity beliefs,quality of doctor-patient communication,course of endocrine therapy received,hope,average monthly household income,and the drug overuse beliefs.Clinical nurses should pay attention to different beliefs of patients and formulate targeted interventions for specific groups to improve the belief of breast cancer patients to take medicine,thereby improving their medicine compliance and promoting their health recovery. |
