| Purpose:By analyzing the factors that influence the quality of life of breast cancer patients treated with endocrine therapy,screening out the risk factors and protective factors,so as to provide a basis for the development of personalized diagnosis and treatment measures in the future.Furthermore,the clinical efficacy and safety of Shugan Jianpi Decoction in the treatment of breast cancer patients with endocrine therapy were further observed to provide reference for clinical TCM treatment of breast cancer patients with endocrine therapy.Method:Part Ⅰ: By referring to relevant literature and consulting experts in this field,a general information questionnaire is designed according to the survey requirements,and the patient’s demographic data and related medical history are collected:Name,age,registration number,contact phone number,marital status,educational level,working status,clinical stage,pathological type,operation method,history of targeted therapy,radiotherapy,chemotherapy,endocrine drug category,endocrine medication duration and TCM medication duration,using a questionnaire to record patients’ quality of life.First,univariate analysis was conducted on the factors that might affect the quality of life of patients,and multivariate Logistic regression analysis was conducted on the factors that were statistically significant after univariate analysis,so as to screen out the independent influencing factors on the quality of life of patients with breast cancer treated with endocrine therapy.Part Ⅱ: According to the clinical research on standard,The patients in the endocrine treatment stage of breast cancer with "Liver stagnation and spleen deficiency syndrome" were collected,On the basis of endocrine therapy,Shugan Jianpi Decoction is combined for before-after study in the same patients,The improvement of quality of life score,TCM symptom score,KPS score,tumor markers(CA153,CEA)were observed after treatment for 8weeks.Results:Part Ⅰ: Appropriate patients were selected according to the standard of intake and discharge.A total of 215 questionnaires were sent out and 211 were recovered.The effective rate of the questionnaires was 98.1% and the average score of quality of life was(105.00±4.97),according to the mean of quality of life score,the patients were divided into two groups: high quality of life and low quality of life.Univariate analysis results showed that there were statistically significant differences in the quality of life scores among patients with different ages,education level,working status,operation methods,endocrine drug categories,endocrine medication duration and TCM medication duration(P<0.05).Marital status,clinical stage,pathological type,history of radiotherapy,history of chemotherapy and history of targeted therapy had no significant effect on the quality of life of patients(P>0.05).Multiariable Logistic regression analysis results showed that the age,the duration of Chinese medicine,statistically significant difference(P<0.05),is endocrine therapy of breast cancer patients quality of life of the independent factors,including age is a risk factor for endocrine therapy of breast cancer patients quality of life,traditional Chinese medicine drug use time is to protect the quality of life factors;There was no statistical significance in education level,working status,operation method,endocrine medication category and endocrine medication duration(P>0.05).Part Ⅱ: According to the standard of the clinical study,40 cases of breast cancer patients treated with endocrine therapy of "Liver stagnation and spleen deficiency syndrome" were collected,1 case was excluded,and 39 cases were finally included.The results showed that the total score of quality of life,physiological status score and emotional status score of the patients were improved compared with those before treatment,with statistical difference(P<0.05).There was no statistical significance in social and family status score,functional status score and additional concern score before and after treatment(P>0.05).Compared with TCM syndrome,the total score of TCM syndrome decreased after treatment compared with before treatment,with statistical difference(P<0.05).The total effective rate of TCM syndrome was 82.1%.The scores of single symptom of TCM before and after treatment were further compared.The results showed that after treatment,the scores of epigastric/hypochoric distension pain,abdominal distension,loose stool,mood depression/irritability,heave a deep sigh,abdominal pain and diarrhea were lower than those before treatment(P<0.05).There was no significant difference between the two groups before and after treatment(P>0.05).After treatment,the KPS score was higher than that before treatment(P<0.05),and the effective rate was 56.5%.Comparison of tumor markers showed that CA153 and CEA of 39 patients were in normal range before and after treatment.There was no new bone marrow suppression and liver and kidney function damage during the treatment.Conclusion:1.Age and duration of Chinese medicine use were independent influencing factors of quality of life of breast cancer patients treated with endocrine therapy.Age is a risk factor for the quality of life of patients treated with endocrine therapy.The older the patients are,the lower the quality of life of the patients.The duration of taking traditional Chinese medicine is a protective factor for the quality of life of patients in this period.2.Shugan Jianpi Decoction can improve the quality of life of breast cancer patients treated with endocrine therapy,and it has a significant effect in improving the physiological and emotional conditions of patients.3.Shugan Jianpi Decoction in the treatment of breast cancer patients treated with endocrine therapy,can effectively relieve patients with epigastric/hypochoric distension pain,abdominal distension,loose stool,mood depression/irritability,heave a deep sigh,abdominal pain that diarrhea,diarrhea after pain reduction and other symptoms of liver depression and spleen deficiency,the effect is significant.4.Shugan Jianpi Decoction can improve the patient’s KPS score.5.Shugan Jianpi Decoction has definite curative effect,good safety and is worth popularizing. |
