Objective: Through clinical examination and comparison,To explore more optical identification characteristics of Gastric intestinal metaplasia(GIM)under WLI(white light imaing,WLI)and LCI(linked color imaging(LCI),Combined application and research analysis were carried out to optimize the screening process of GIM,analyze and compare the identification ability of WLI and LCI to GIM and the diagnostic effectiveness of doctors of different years,and initially establish an efficient diagnostic mode process for GIM to assist clinicians to better serve patients.Methods: A total of 200 outpatient and inpatient patients with abdominal distension,abdominal pain,acid reflux and other symptoms who visited the Department of Gastroenterology in Xinfu Road Medical Area and Midong People’s Hospital of our Hospital from January 2021 to February 2023 for endoscopic examination were included in this study,including 103 males and 97 females,with an average age of 53.8±12.7 years.Gastroscopy(WLI and LCI)examination was performed by senior physicians with LCI experience,and pictures of the areas of interest were retained and biopsied,and then the optical characteristics of WLI and LCI in the endoscopic static images of the gastric mucosa in the areas of interest were observed,analyzed and determined by the endoscopicists in the senior group and the junior group,respectively,and the determined results were compared and statistically analyzed with the pathological gold standard.Results:1.A total of 158 suspicious GIM lesions meeting the requirements were collected under the LCI mode,of which 125 were lilac lesions.The pathological diagnosis was 97 GIM lesions of different degrees,suggesting that lilac manifestations were significantly correlated with GIM,and this color sign could be used as an effective optical signal of GIM in endoscopic screening.2.Compared with WLI mode,LCI mode has better sensitivity for GIM lesions existing in gastric mucosa(93.2%vs80.4%),which is conducive to the detection of lesions in routine endoscopy.3.By comparing the recognition of GIM by physicians with high and low seniority in the two modes,the analysis showed that compared with the WLI mode,LCI mode was helpful to further improve the detection rate of lesions by physicians with high seniority,and the detection rate of physicians with low seniority was close to that of physicians with high seniority in the LCI mode,narrowing the gap between them.Conclusion: 1.Under LCI,the special signal " Lavender coler sign" has a good suggestive effect on GIM,which is obviously consistent with pathological biopsy,and is expected to reduce unnecessary biopsy sampling.2.Observation in LCI mode is less affected by the lesion’s mucosal background,which still has a good diagnostic value for lesions with HP infection and atypical lesion morphology.3.LCI technology can improve the diagnostic accuracy of primary physicians and help primary physicians to screen the lesions.
