AIM:To evaluate the diagnostic value of the Linked Color Imaging(LCI)model for gastrointestinal metaplasia,color differences were objectively evaluated using CIE1976L* a * b* color space were used to distinguish the boundary between intestinal metaplasia and surrounding mucosa color in LCI mode.The RGB color model was used to objectively evaluate the difference of color value among different degrees of intestinal metaplasia in LCI mode.The color value was used to establish a function to distinguish the severity of intestinal metaplasia,to evaluate the diagnostic value of LCI model for the severity of intestinal metaplasia,to establish a computerized endoscopic diagnostic system based on the image characteristics of intestinal metaplasia under the LCI model.Methods:A total of 61 patients(both inpatient and outpatient)were enrolled in the First Affiliated Hospital of Jinan University.They were examined by white light endoscopy(WLI)and LCI mode,and histological examination was performed for suspicious lesions.The endoscopic diagnosis of lesions with purple appearance observed in the nonmagnification mode of LCI was intestinal metaplasia,and the endoscopic diagnosis results were compared with the histological examination results.CIE1976L* a * b * color spatial analysis was used to compare the color difference(ΔE)between intestinal metaplasia and surrounding mucosa in LCI and WLI modes The pixel brightness values of purple intestinal metaplasia under LCI non-amplification mode were analyzed and compared using RGB color model,and the pixel brightness values were used to predict the severity of intestinal metaplasia under LCI mode.LCI images related to intestinal metaplasia were trained for pathological diagnosis,lesion models were obtained,and a computerized endoscopic diagnostic system was preliminarily established.Results:1.A total of 39 purple lesions was observed in LCI mode.The pathological diagnosis was 34 intestinal metaplasia.The coincidence rate with pathological diagnosis was 91.9%(34/37).2.The average values of ΔE between intestinal metaplasia and surrounding mucosa in LCI group were higher than those in WLI group(12.74±5.68 vs 7.56±4.25,P=0.00),LCI show color difference(ΔE)better than WLI,Then We analyzed the L value,b* value,a*value sample regions between intestinal metaplasia and surrounding mucosa in LCI images,The a* and b* show obvious difference among them,The ROC curve was used to find the critical value of intestinal metaplasia and surrounding mucosal color in LCI mode.The critical value of intestinal metaplasia and peripheral mucosa on a* axis under LCI microscope was 17.75,Sensitivity was 0.703,specificity was 0.703;The critical value between the two on the b* axis under LCI the mirror was 23.5,The sensitivity and specificity were low.3.RGB color model is used to objectively evaluate LCI image color values between different degree of intestinal metaplasia,the color value(G)there exists significant difference between different degree of intestinal raw(P = 0.001),and R and B value of no difference(P = 0.219,P = 0.106),Fisher function was applied to 34 pieces of LCI image IM degree classification prediction,the result shows there are 28 observation is correct,six observation is wrong,82.4%(28/34)of the original observation was sentenced to misjudgement was 17.6%(6/34).4.148 LCI images of intestinal metaplasia were trained to obtain lesion models,and a computerized image diagnostic system was preliminarily established.Then 72 LCI images were used to evaluate the diagnostic efficacy of the system.The sensitivity,specificity and accuracy of the system were 35.1%、82.9% and 58.3% respectively.Conclusion:1.In the non-amplification mode of LCI,purple has a high accuracy in the diagnosis of intestinal metaplasia,and it is easy to identify the color difference between purple and surrounding mucosa.This method is simple and can be used as a screening method for high-risk patients with intestinal metaplasia.2.According to the pixel brightness value of intestinal metaplasia related images in non-amplification mode of LCI,the severity of gastrointestinal metaplasia can be predicted.3.Based on the image characteristics of intestinal metaplasia in LCI non-magnification mode,a computerized image iagnostic system was preliminarily established to provide an innovative screening scheme for gastric cancer.
