Objective:To investigate the correlation between serum uric acid level(SUA)and central arterial pressure and carotid intimal media thickness(CIMT)in patients with essential hypertension.Methods:The retrospective study included 779 patients with essential hypertension diagnosed in the Department of Hypertension of the First Affiliated Hospital of Xinjiang Medical University.Using the quartile method,the patients were divided into198 in Q1,191(SUA≤282μmol/L)in Q2(282 μmol/L <SUA ≤334 μmol/L),196 in Q3(334 μmol/L<SUA ≤397 μmol/L),and 194 in Q4(SUA>397μmol/L).The general clinical data,laboratory data,central arterial pressure,CIMT and other relevant indicators were compared,and the relationship between serum uric acid,central arterial pressure and CIMT and its influencing factors were analyzed.Results:There were no significant differences in gender,systolic blood pressure,diastolic blood pressure,heart rate,fasting blood glucose,total cholesterol,LDL and urea nitrogen were compared among 4 groups(P >0.05).There were statistically significant differences in age,BMI,triglyceride,HDL and creatinine(P <0.05).Using age,BMI,triglyceride,HDL and creatinine as covariables,covariance analysis showed that CSP,CPP,AIx and CIMT were significantly different among the four groups(P >0.05),from group Q1 to group Q4,with SUA increasing step by step,the above indexes also increased gradually,especially in group Q4;pearson correlation analysis showed that serum uric acid was positively correlated with CSP,CDP,CPP,AIx and CIMT(P <0.001).Multivariate linear regression was performed to control baseline risk with central arterial pressure as the dependent variable and uric acid and baseline data with differences between groups as the independent variable.The results showed that uric acid was an influencing factor of CSP,CPP and AIx(P <0.05).The CIMT was divided into normal(< 1mm)and thickened groups according to thickness,and binary logistic regression was performed.First,univariate logistics regression analysis was performed.The results showed that age,creatinine and total cholesterol were risk factors for thickened CIMT in hypertension patients(P <0.05).In the univariate logistics analysis of SUA level grouping and CIMT in hypertensive patients,group Q1 was taken as the control group.The results showed that Q2 VSQ1(OR=1.4,95%CI: 0.76-2.56,P=0.283),Q3 VS Q1(OR=1.72,95%CI: 0.95-3.09,P=0.071),Q4 VS Q1(OR=2.01,95%CI:1.13)~3.56,P=0.018)),high SUA was a risk factor for CIMT thickening.Multivariate Logistic regression analysis was performed after adjusting for meaningful variables in univariate analysis,and the results showed that high SUA in hypertensive patients was an independent risk factor for CIMT thickening.Conclusion:In this study,SUA level was positively correlated with CIMT in patients with hypertension.SUA level(>334μmol/L)was an independent risk factor for CIMT thickening,and serum uric acid affected the increased levels of CSP,CPP and AIx.
