Objective:To study the effect of different parameter combinations of fractional CO2 laser in the hyperplastic stage of pediatric burn.Materials and Methods:A total of 160 pediatric patients with hypertrophic scar after limb burn were treaed in our hospital from 2017 to 2020 and randomly divided into four parameter groups(n=40).The patients were treated with ablative fraction carbon dioxide laser,once every 10 weeks.During the interval of laser treatment,Compound Heparin Sodium and Allantoin Gel(Contractubex)was applied externally,tid,and elastic cover or elastic bandage is attached to the affected limb.Vancouver scar scale score was performed before each laser treatment,the initial score was scored before the first treatment,and the two were scored separately,with an average score.The patients were followed up for 4 times and scored.The differences between the scores of each parameter group after treatment and the first time were compared,the scores of different treatment densities under the same energy were different after treatment,and the scores of different energies under the same density were compared.The conditions of bleeding and pigmentation in each parameter group were compared.Results:Compared with the first score,there was a difference before the fourth treatment in the 17.5/5%parameter group,a difference in the 25mj/5%parameter group before the third treatment and the fourth treatment,and a difference before the second treatment between the 17.5mj/10%parameter group and the 25m/10%parameter group.At low density,the high-energy and low-energy group showed a score difference after the first three treatments;in the high-density group,the high-energy group was significantly different from the low-energy group only after the first treatment.There was no significant difference between the high-energy and high-density group(25mj/10%)and the low-energy and low-density group(17.5mj/5%),but there was no significant difference between the two groups before the fourth treatment.Statistics of the incidence of bleeding and pigmentation in each group:under the same density,the score of the low-energy group was lower than that of the high-energy group;under the same energy,there was no significant difference between the low-density group and the high-density group.In terms of the intervention effect of energy and density on scar hyperplasia in children after burn,the increase of energy did not significantly increase the therapeutic effect,but the number of cases of pain and hemorrhagic pigmentation increased significantly.The subjective expression of pain in children is not clear,so this study does not count the differences of pain among the parameter groups.According to the treatment experience of adults,the increase of energy and density can aggravate the pain.
