| Objective:1.Based on a retrospective analysis of 120 patients with hypertension caused by hyperactivity of liver fire,the medication characteristics of Traditional Chinese Medicine treatment were summarized,in order to provide more reference for clinical treatment.2.Using the network pharmacology methods to explore the therapeutic mechanisms of core prescriptions in treatment of hypertension caused by hyperactivity of liver fire.Methods:1.Retrospective analysis of case data of patients with liver fire hyperactivity type hypertension who attended the outpatient clinic in Cardiology Department of the First Affiliated Hospital of Heilongjiang University of Traditional Chinese Medicine from June 2020 to December 2022.Before and after treatment were statistically analyzed by Excel table and SPSS software on blood pressure level,medication frequency,association rules,etc.2.The TCMID database obtains the core drug active ingredients and performs ADME screening,and then predicts the target in the Swiss-Target Prediction database,predicts the disease target in the Genecards database,and intersects the drug and the disease target.Using Cytoscape software to build a "drug-active ingredient-target" network;String database to build PPI(protein interaction)network;The Metascape platform was used to analyze GO function and KEGG pathway for drug targets.Result:1.Total of 120 patients with liver fire hyperactivity type hypertension,75 females and 45 males,the average age is 49.28±12.26 years old.About three quarters of patients have other diseases.The top five comorbid diseases are:coronary heart disease(31.67%),cervical spondylosis(25%),cerebral infarction(17.5%),diabetes(14.17%)and dyslipidemia(10.83%).2.Before treatment,the median systolic blood pressure in 120 patients was 155.50mmHg,and the median diastolic blood pressure was 91.00mmHg;After treatment,the median systolic blood pressure was 140.00 mmHg,and the median diastolic blood pressure was 82.50 mmHg,with a statistically significant difference(P<0.05).3.It contains 295 prescriptions,involving 175 traditional Chinese medicines.Often appears the properties are cold and bitter taste,mainly meridians are liver,lung,spleen,and heart;The top 10 traditional Chinese medicines were Tall Gastrodia Tuber,Gambir Plant,Chinese Thorowax Root,White Paeony Root,Atractylodes macrocephala,Poria cocos,Chrysanthemum,Fossilizid,Oyster Shell,and Cape Jasmine Fruit.High frequency drugs such as calm the liver and quench the wind,clear heat,activate blood and remove stasis are commonly used;Commonly used drug combinations include Tall Gastrodia Tuber-Gambir Plant,Chinese Thorowax Root-White Paeony Root,etc;The association rules with high confidence include "Chinese Thorowax Root→Bitter Orange","Chinese Thorowax Root→Turmeric Root-tuber,White Paeony Root","Gambir Plant→Medicinal Cyathula Root,Chrysanthemum,Tall Gastrodia Tuber",etc;Cluster analysis yielded 7 drug combinations.4.The network pharmacological analysis of core perscription drugs showed that 272 targets for drug treatment were obtained,and the protein interaction network showed that the core targets were SRC,MAPK3,MAPK1,STAT3,JUN,AKT1,etc.KEGG pathway enrichment analysis showed that core prescription drugs in the treatment of essential hypertension improved blood pressure levels through lipid and atherosclerosis,fluid shear stress and atherosclerosis,AGE-RAGE signaling pathway in diabetic complications,neuroactive ligand-receptor interactions,and calcium signaling pathways,etc.Conclusion:1.In this study,the traditional Chinese medicine used to treat hypertension with excessive liver fire syndrome is mainly composed of drugs for calming the liver and calming the wind,clearing heat,and promoting blood circulation and resolving blood stasis.The core prescription for treatment is:Tall Gastrodia Tuber,Gambir Plant,Chinese Thorowax Root,White Paeony Root,Atractylodes macrocephala,Poria cocos,Commonly used drug combinations include Tall Gastrodia Tuber-Gambir Plant,Chinese Thorowax Root-White Paeony Root,etc.2.Network pharmacology studies have shows that the mechanism of core prescriptions drugs in the treatment of this disease may be achieved through the regulation of lipid and atherosclerosis,fluid shear stress and atherosclerosis,AGE-RAGE signaling pathway in diabetic complications,neuroactive ligand-receptor interaction,calcium signaling pathway and other signaling pathways. |
