Therapeutic Effect Of Scraping Along Meridians On Thyroid Nodules With Qi Stagnation And Phlegm Coagulation Syndrome

Objective:The purpose of the study is to observe the nursing effect of scraping along meridians on patients who have thyroid nodules with Qi stagnation and phlegm coagulation syndrome.Impact of this therapy on clinical symptoms and TCM syndrome of thyroid benign nodules has been comprehensively evaluated,as well as its effectiveness,safety and availability.Technical operation methods of this scraping therapy should be standardized further,in order to provide objective basis and new ideas for TCM nursing and treatment of thyroid nodule diseases.Methods:In this study,64 subjects who met the diagnostic criteria,inclusion criteria and exclusion criteria were recruited in the thyroid ultrasound screening clinic of the First Affiliated Hospital of Heilongjiang University of TCM from September 2021 to September 2022.All subjects were randomly divided into control group(n=32)and experimental group(n=32).Nursing routine was fulfilled in the two groups for 12 weeks.Besides,the control group received oral administration of levothyroxine sodium daily while the experimental group received scraping therapy once a week.After 12 weeks,TCM syndrome scores,maximum diameter of thyroid nodules,thyroid function indicators,quality of life questionnaire(THy PRO39)scores and safety factors were compared between the two groups before and after intervention.All data were statistically analyzed by SPSS23.0 software to comprehensively evaluate the effectiveness of clinical intervention.Results:1.Baseline level:Before intervention,the general information,TCM syndrome scores,thyroid nodule size,thyroid function indicators,and quality of life questionnaire scores of the two groups were compared.The difference was not statistically significant(P>0.05),which indicated that the baseline of the two groups was basically the same and comparable.2.Clinical efficacy:After intervention,the total effective rate of clinical efficacy in scraping group was 86.7%,which was significantly higher than that in pharmaceutical group(63.3%),with a statistically significant difference(P<0.05).3.TCM syndrome score and curative effect:Both groups had a decrease in TCM syndrome scores compared to before intervention(P<0.05),and the reduction in the scraping group was greater than that in the drug group(P<0.05).The total effective rate of TCM syndrome efficacy of scraping group was 76.7%,which was better than that of the drug group(50.0%),and the difference was statistically significant(P<0.05).4.Changes in thyroid nodule size:After intervention,both maximum diameter of thyroid nodules of the two groups decreased with a statistically significant difference(P<0.05).The reduction of thyroid nodules in the scraping group was significantly more than that in the drug group(P<0.05).The maximum diameter of thyroid nodules of the two group was reduced compared with themselves before intervention(P<0.05).However,there was no significant change in thyroid nodule size of control group(P>0.05).5.Thyroid function indicators:After intervention,the levels of FT3 and FT4 increased and TSH decreased in the drug group whose differences were statistically significant compared with those before intervention and after treatment in the scraping group(P<0.05),while there was no statistically significant difference in the scraping group before and after treatment(P>0.05).6.THyPRO39 quality of life score:After intervention,the quality of life scores of both groups decreased and the differences within the group were statistically significant(P < 0.05).After intervention,the improvement of the score in the scraping group was significantly better than that in the drug group(P < 0.05).7.Safety evaluation:During the entire study period,vital signs of subjects in the two groups remained basically stable,and no serious adverse reactions or other abnormal changes occurred.The safety evaluation grade was level 1.Conclusion:Scraping along meridians can improve the clinical symptoms of patients with thyroid nodules Qi stagnation and phlegm coagulation syndrome,reduce the size of nodules and improve the quality of life of patients,with good clinical efficacy.At the same time,it has no effect on patient’s thyroid function and thyroid antibodies,with no obvious adverse reactions and side effects.This method is simple,safe and effective which has application and promotion value in clinical practice.Its mechanism of action is worth further research.
