| Objective: To compare and analyse changes in characteristic dermoscopic images of vitiligo before and after conditioning treatments(mature,clearing therapies)to provide a basis for the evaluation of indicators for the treatment of abnormal mucinous vitiligo.Methods: 1.136 patients with astringent mucinous type,salty mucinous type and odorless mucinous type vitiligo who met the inclusion and exclusion criteria and were hospitalized at our dermatology centre between October 2020 and March 2023 were selected for this study,patients enrolled are treated with the appropriate maturing and clearing agents(conditioning therapy)depending on the abnormal body fluid type,dermoscopic monitoring of skin lesions before and after conditioning treatments,observation of characteristic dermoscopic image changes at the lesion,changes such as background colour,vascular morphology,border conditions,hair,hair follicles,pigmentation,and textural features of the skin surface are analysed statistically.Results:Examination by dermoscopy reveals the following: Odorless mucinous type vitiligo:(1)After conditioning treatment,Milky white and porcelain white background colours appear significantly less frequently,Significantly more frequent occurrences of off-white and red-white background colours,the difference was statistically significant(P<0.05);(2)After conditioning treatment,significantly more frequent occurrence of well-defined boundaries,significantly less frequent occurrence of blurred boundaries,the difference is statistically significant(P < 0.05);(3)After conditioning treatment,deepening of peridermal pigmentation,a significant increase in the frequency of easily visible pigmented islands,the difference was statistically significant(P < 0.05);(4)After conditioning treatment,peripheral punctate,irregular hypopigmented spots,patches of pigmentation visible in areas of white spots,dull skin colour on white patches,significantly less frequent appearance of perifollicular pigment residue,the difference was statistically significant(P < 0.05);Astringent mucinous type vitiligo:(1)After conditioning treatment,milky white background colours appear significantly less frequently,the difference was statistically significant(P<0.05);(2)After conditioning treatment,significantly more frequent occurrence of well-defined boundaries,significantly less frequent occurrence of blurred boundaries,the difference is statistically significant(P < 0.05);(3)After conditioning treatment,deepening of peridermal pigmentation,a significant increase in the frequency of easily visible pigmented islands,the difference was statistically significant(P<0.05);(4)After conditioning treatment,peripheral punctate,irregular hypopigmented spots,patches of pigmentation visible in areas of white spots,Significantly less frequent appearance of dull skin colour at white spots,the difference was statistically significant(P<0.05);Salty mucinous type vitiligo:(1)After conditioning treatment,milky white background colours appear significantly less frequently,significantly more frequent occurrences of porcelain white,off-white and red-white background colours,the difference is statistically significant(P<0.05);(2)After conditioning treatment,significantly more frequent occurrence of well-defined boundaries,significantly less frequent occurrence of blurred boundaries,the difference is statistically significant(P < 0.05);(3)After conditioning treatment,deepening of peridermal pigmentation,a significant increase in the frequency of easily visible pigmented islands,the difference was statistically significant(P < 0.05);(4)After conditioning treatment,peripheral punctate,irregular hypopigmented spots,patches of pigmentation visible in areas of white spots,Significantly less frequent appearance of dull skin colour at white spots,the difference was statistically significant(P<0.05);Conclusion: 1.The dermoscopic features of the three abnormal humoral vitiligo types after treatment are:(1)Odorless mucinous type vitiligo: After conditioning treatment,(1)Milky white and porcelain white background colour,blurred borders,peripheral dots,irregular hypopigmented patches,patches of pigmented residue visible in the white area,dull skin colour in the white area,significantly less frequent appearance of perifollicular pigmentation;(2)A marked increase in the frequency of pale white and reddish-white coloured backgrounds,well-defined borders,deepened peridermal pigmentation and easily visible islands of pigmentation;(2)Astringent mucinous type vitiligo: After conditioning treatment,(1)A milky white colour background,blurred borders,peripheral dots,irregular hypopigmented patches,patches of pigmented residue visible in the area of the white patches,and a significant reduction in the frequency of dull skin colour at the white patches;(2)A marked increase in the frequency of well-defined borders,deepened peridermal pigmentation and easily visible islands of pigmentation;(3)Salty mucinous type vitiligo: After conditioning treatment,(1)A milky white colour background,blurred borders,peripheral dots,irregular hypopigmented patches,patches of pigmented residue visible in the area of the white patches,and a significant reduction in the frequency of dull skin colour at the white patches;(2)A marked increase in the frequency of porcelain,pale and reddish-white background colour,well-defined borders,deepened peridermal pigmentation and easily visible islands of pigmentation;2.Dermoscopy can be used as an objective monitoring tool to evaluate the efficacy of vitiligo treatment and to determine the healing of the disease;Characteristic dermoscopic images show,that Uyghur medical conditioning treatment(mature,clearing therapy)has significant clinical efficacy for different types of vitiligo and can be applied clinically. |
