Objective:Calculate the distribution and composition of traditional Chinese medicine syndrome types in overweight and obese PCOS patients,Compare the differences in serological indicators among overweight and obese PCOS patients with different traditional Chinese medicine syndrome types,Analyze the correlation between different traditional Chinese medicine syndrome types and different serological indicators in overweight and obese PCOS patients.Method:PCOS patients who visited the Gynecology Clinic of the First Affiliated Hospital of Heilongjiang Traditional Chinese Medicine University from September 2014 to June 2022 were included,After obtaining the consent of the patient,a questionnaire survey related to PCOS traditional Chinese medicine syndrome types will be conducted,and the patient’s anthropometric indicators(height,weight)and serological indicators(sex hormones,blood sugar,blood lipids)will be measured,and the data will be organized and recorded.Apply SPSS 25.0 for statistical analysis.Divide patients into overweight and obese groups for analysis,The distribution characteristics of the research objects are expressed in numbers(percentages);If the continuous measurement data conforms to the normal distribution,the independent sample T test(two groups)or one-way anova(multiple groups)shall be used for comparison,described as X ± s;If it does not conform to normal distribution,nonparametric test shall be used,Using Mann Whitney U test(two groups)or Kruskal Wallis H test(multiple groups)for analysis,described as median(interquartile range);The correlation between traditional Chinese medicine syndrome types and serological indicators was analyzed using binary logistic regression,and the results were expressed as ratio ratios(OR).All P<0.05 are considered statistically significant.Results:1.A total of 1017 patients with PCOS were included in the study,According to BMI,among these 1017 patients,underweight accounted for 4%(n=41),normal weight accounted for 22.6%(n=230),overweight accounted for 35.6%(n=362),and obesity accounted for 37.8%(n=384);According to traditional Chinese medicine syndrome types,among these patients,spleen deficiency and phlegm dampness accounted for 14.1%(n=143),kidney deficiency and liver stagnation accounted for 29.3%(n=298),kidney deficiency and blood stasis accounted for 19.4%(n=197),and phlegm stasis combined accounted for 37.3%(n=379).2.The traditional Chinese medicine syndrome types of overweight group,from high to low,are phlegm and blood stasis(n=132,36.5%),kidney deficiency and liver depression(n=103,28.5%),kidney deficiency and blood stasis(n=78,21.5%),and spleen deficiency and phlegm dampness(n=49,13.5%);The TCM syndrome types of the obesity group,from high to low,are phlegm and blood stasis(n=190,49.5%),kidney deficiency and liver depression(n=85,22.1%),spleen deficiency and phlegm dampness(n=64,16.7%),kidney deficiency and blood stasis(n=45,11.7%).There is a statistically significant difference in the proportion of traditional Chinese medicine syndrome types between the two groups(P<0.05).3.Comparison of serological indicators: The levels of LH,DHEAS,SHBG,HDL,and Apo A in the overweight group were higher than those in the obese group;The 120 min glucose,FINS,the 120-minute insulin,TC,TG,LDL,Apo B levels in the obese group were higher than those in the overweight group,The above differences were statistically significant(P<0.05).4.Comparison of differences in syndrome types among overweight group:the 120-minute glucose level in the syndrome of kidney deficiency and liver stagnation is higher than that in the syndrome of kidney deficiency and blood stasis,The difference was statistically significant(P<0.05).5.Comparison of differences in syndrome types among obese groups:The FSH level in spleen deficiency and phlegm dampness syndrome is higher than that in phlegm stasis syndrome;the 120-minute insulin level of kidney deficiency and blood stasis syndrome is higher than that in spleen deficiency and phlegm dampness syndrome,The difference was statistically significant(P<0.05).6.The overweight group correlation analysis showed that: Kidney deficiency and blood stasis syndrome are positively correlated with TC,The difference was statistically significant(P<0.05).7.The obese group correlation analysis showed that: The syndrome of spleen deficiency and phlegm dampness has a positive correlation with FSH and a negative correlation with the 120-minute insulin;Kidney deficiency and blood stasis syndrome are positively correlated with SHBG and the120-minute insulin;There is a negative correlation between phlegm and blood stasis syndrome and SHBG,the 120-minute glucose;The syndrome of kidney deficiency and liver depression is positively correlated with the 120-minute glucose,The above differences were statistically significant(P<0.05).Conclusions:1.The manifestation rate of phlegm and blood stasis in overweight and obese PCOS patients is the highest,followed by kidney deficiency and liver depression.2.The level of serum indicators in overweight and obese PCOS patients is related to the degree of weight gain.The higher the degree of weight gain,the more obvious the abnormality of glucose and lipid metabolism;LH and DHEAS levels have slightly improved.3.There is basically no specific difference in the syndrome types of overweight PCOS patients between sexual hormones and glucose and lipid metabolism indicators;There is also no correlation between its syndrome types and hormone and glucose lipid metabolism indicators.In overweight PCOS patients,the clinical significance of serological indicators for distinguishing traditional Chinese medicine symptoms is not significant.4.In obese PCOS patients,The higher the FSH level,the lower the120-minute insulin level,and the more likely it is to manifest as spleen deficiency and phlegm dampness syndrome;The higher the levels of the120-minute insulin and SHBG,the more likely it is to manifest as kidney deficiency and blood stasis syndrome;The lower the SHBG and the120-minute glucose levels,the more likely it is to manifest as phlegm stasis syndrome;The higher the 120-minute glucose level,the more likely it is to manifest as kidney deficiency and liver depression syndrome.
