Clinical Observation On The Treatment Of Chronic Kidney Disease Stage 3-5 (Non-dialysis) Patients Based On Up-down Open-close Pivot Theory

Objective:To explore the connotation of the theory of "Up-down open-close pivot theory" by observing the clinical efficacy of Shengjiang Anshen decoction in the treatment of patients with chronic kidney disease(CKD)stage 3-5(non-dialysis),The clinical efficacy of Up-down open-close pivot theory in the treatment of chronic kidney disease with spleen-kidney qi deficiency and turbidity and blood stasis is further verified.Methods:In this study,60 non-dialysis outpatients(30 cases in the treatment group and 30 cases in the control group)who met the diagnostic criteria of stage 3-5 chronic kidney disease(spleen and kidney qi deficiency and turbidity and blood stasis syndrome)in the Department of Nephrologyof the First Affiliated Hospital of Heilongjiang University of Chinese Medicine from December 2021 to December 2022 were selected as the research objects.On the basis of conventional treatment,the control group was treated with Shenshuaining capsule,and the treatment group was treated with Shengjiang Anshen decoction.The efficacy indexes(the scores of TCM syndromes,serum creatinine,urea nitrogen,glomerular filtration rate,serum cystatin C)and statistically analyzed before treatment and after 6 weeks of treatment.Results:1.After treatment,the levels of serum creatinine,urea nitrogen and serum cystatin C in the two groups were lower than those before treatment.The serum creatinine level,urea nitrogen level and serum cystatin C level of the treatment group were lower than those of the control group after treatment(P<0.05),the average level of urea nitrogen(P<0.05)and serum cystatin C(P<0.05),the difference was statistically significant.After treatment,the glomerular filtration rate of the two groups increased(P<0.05),and the difference was statistically significant,and the glomerular filtration rate of the treatment group was higher than that of the control group after treatment(P<0.05),and the difference was statistically significant.2.After treatment,the clinical symptoms of the two groups were both improved,and the TCM syndrome scores were decreased(P<0.05).While the improvement of the main symptoms in the treatment group was better than that in the control group(P<0.05).In terms of the improvement of secondary symptoms,the improvement of stool rotten and sticky mouth in the treatment group was better than that in the control group(P<0.05).The total score of TCM symptoms in the treatment group was lower than that in the control group(P<0.05).The results showed that the treatment group was better than the control group in improving the TCM symptom score(P<0.05).3.After treatment,the total effective rate of the treatment group was 76.67%,the total effective rate of the control group was 68.97%,the therapeutic effect of the treatment group was better than that of the control group.Conclusions:1.The treatment based on the theory of "Up-down open-close pivot" can effectively reduce the levels of serum creatinine,urea nitrogen and cystatin C,and improve the glomerular filtration rate in patients with stage 3-5 chronic kidney disease(non-dialysis)of spleen-kidney qi deficiency and turbidity and blood stasis.2.Based on the theory of "Up-down open-close pivot",the treatment of chronic kidney disease stage 3-5(non-dialysis)patients with spleen and kidney qi deficiency and turbidity and blood stasis type can improve the clinical discomfort symptoms of patients such as shortness of breath and lazy speech,limb fatigue and weight,unsolid stool,and sticky mouth.3.Based on the theory of lifting,lowering,opening and closing shu,the treatment of chronic kidney disease stage 3-5(non-dialysis)patients with spleen and kidney qi deficiency and turbidity and blood stasis type is effective.
