Clinical Experience Of Xu Jingshi,a Master Of Chinese Medicine,in The Treatment Of Digestive Tract Tumors

Objective: With the help of data mining technology,this paper probes into the law of drug use of Dr.Xu Jingshi,a great master of Chinese medicine,in the diagnosis and treatment of digestive tract tumors,and summarizes Mr.Xu’s clinical treatment experience and preliminary exploration of its academic theory.Methods: We collected and sorted the medical records of Mr.Xu Jingshi from January2016 to December 2022,and screened out 165 medical records in accordance with the medical records,which were 329 times in total.First,the medical records that meet the requirements were entered into Excel 2016 form,and then into the clinical medical records collection system provided by Anhui Traditional Chinese medicine.During the period,the medical records were standardized,and after two people checked that the information of the medical records was correct,it was introduced into the clinical medical record data processing system.SPSSClementine Client11.1 software and Apriori algorithm were used to analyze the data in the medical record,including frequency analysis,association rule analysis,cluster analysis and complex network analysis,finally,the results of the excavation were analyzed and summarized to explore the rule of Xu Jingshi’s diagnosis and treatment of digestive tract tumors.Results:(1)Among the 165 cases of digestive tract tumor,96 cases were male,accounting for58.18%,69 cases were female,accounting for 41.82%,the ratio of male to female was1.39:1.The age group of 61-70 years old was the most(27.88%),the age group of51-60 years old was the second(21.21%).(2)A total of 329 visits were made in 165 patients with digestive tract tumors,which were in order from high to low: liver(90 visits),27.35%;stomach(85 visits),25.83%;esophagus(53 visits),16.10%;colon(47 visits),14.28%,6.77% and 10.61% of the cases were rectum(22),pancreas,gallbladder,bile duct and duodenum respectively The first 7 types of tongue appearance with high frequency were dark red,red,thin yellow,thin white,dark,yellow and greasy,and slippery The first 5 cases with high frequency of pulse were as follows: fine pulse chord,fine pulse chord,weak pulse chord,number of pulse chord;The first 6 syndrome types with higher frequency were:Maladjustment of Zhongzhou,injury of both Qi and Yin,disharmony between liver and stomach,deficiency of vital energy,stagnation of liver-qi and accumulation of damp-heat;The first 10 most frequent symptoms were fatigue,insomnia,dry mouth,full stomach,diarrhea,loose stool,bitter mouth,poor appetite,constipation and abdominal pain.(3)A total of 329 prescriptions were collected and 201 Chinese herbs were used for a total of 3443 times.The frequency of use ≥50 drugs for 19,of which not less than 70 have 13,the 13 medicines were Zhuru,green calyx plum,Gleditsia bungeana,aurantium aurantium,Cynanchum,Dendrobium,semen ziziphi spinosae,liquorice,dried tangerine peel,Paeonia lactiflora,Coptis chinensis,fried millet bud,Ophiopogon japonicus,the total percentage was 44.15%.The frequency of use of cold drugs was the most,accounting for 45.79%,followed by mild and warm drugs,accounting for29.06% and 21.17%,respectively,the most frequently used drugs were sweet(34.14%),followed by bitter,sour and pungent drugs(29.39%),sour(13.39%)and pungent(12.23%),the percentage of lung,liver and spleen meridians was 18.66%,16.01% and13.95%,respectively,secondly,qi-regulating drugs,phlegm-resolving drugs,heat-clearing drugs,sedative drugs,a total of 70.11%.(4)Results of association rules: the frequency of more than 70 Chinese medicine fields were input before and after the term,the minimum confidence level was set to 80%,the minimum support level was set to 10%,a total of 141 eligible drug group,there were 6groups of two-drug combination,44 groups of three-drug combination and 91 groups of four-drug combination.The combination of the two drugs is mainly used for regulating the liver and the stomach,nourishing yin and promoting fluid production,nourishing the lung and stomach,clearing heat and resolving phlegm,lowering adverse effects and the stomach,regulating the stomach and strengthening the spleen,etc.,heat-clearing and phlegm-resolving;the Association of the four drugs mainly embodies the combination of soothing the liver and stomach,supplementing qi and nourishing Yin,Fuzheng ’an medium.(5)After cluster analysis,three core drug groups were obtained:(1)Licorice,white peony,ophiopogon japonicus,Semen Ziziphi spinosae,Coptis chinensis(2)North ginseng,Dendrobium,crane grass(3)Zhuru,green calyx plum,orange,tangerine peel,fried grain bud(6)Through complex network analysis,the key prescriptions were: green calyx plum,north ginseng,bamboo,orchids,dendrobium,jujube seed,fried millet bud,trifoliate orange,Astragalus,liquorice,atractylodes macrocephala,peony root,Ginger Pinellia Ternate,Hedyotis diffusa root,Ophiopogon japonicus.Conclusion:(1)Xu Jingshi thought that the patients with digestive tract tumor had deficiency of qi and excess of the origin and deficiency of the vital energy as the origin.Although the pathological changes mostly affected the spleen and stomach,they were also closely related to the liver and gallbladder.The pathogenesis of digestive tract tumor was mostly a mixture of cold and heat,mixed with virtual and real.(2)In the treatment of digestive tract tumor,we should pay attention to both strengthening health and eliminating pathogenic factors,master the evolution,balance the urgency,take strengthening health as the foundation stone,strengthen the spleen and stomach as the first step to strengthen the health,so as to harmonize the state and strengthen the source of health,aimed at“Righting peace”.(3)The prescription is flexible and changeable,combined with recuperation and balance therapy,self-created“Fu Zheng an Zhong Tang”,which is commonly used in clinical practice.(4)The characteristics of medication are as follows:(1)The medicine nature is peaceful,the sweet taste is mostly,light nimble and flexible,good use invigorates the weak medicine,especially in the Yin medicine is very.(2)The principle of“Nourishing stomach with Gan Ping” was emphasized,and the method of bi-directional regulation was adopted to balance the rise and fall of spleen and stomach,and also to regulate the liver and regulate the lung.(3)Medication follows the“Three bogeys” thought,that is,“Bu jun bu bu,Zi but not greasy,Wen but not dry.(5)Give attention to the diet,daily life and emotional nursing of patients with gastrointestinal cancer,and pay attention to the treatment of mind and body.
