| Objective: Through the observation of the clinical effect of Xugu Huoxue Decoction combined with Proximal Femoral Nail Anti-Rotation(PFNA)internal fixation in the treatment of intertrochanteric fracture of the femur in the elderly,this paper discusses the basis and advantages of Xugu Huoxue Decoction in the treatment of intertrochanteric fracture of the femur,and provides clinical reference for the treatment of intertrochanteric fracture of the femur in the elderly.Methods: In this study,52 elderly patients with intertrochanteric fracture of femur combined with osteoporosis admitted to the Department of Orthopaedic Trauma,the First Affiliated Hospital of Anhui University of Chinese Medicine from March 2021 to May 2022 were selected.All of them met the inclusion criteria of this study,and all of them received PFNA surgery in our hospital.The study subjects were randomly divided into the observation group(26 cases)and the control group(26 cases).After PFNA surgery,the control group was given alfacalcidol soft capsule anti-osteoporosis treatment,0.5 ug/time,1 time/day.After PFNA operation,observation group was given oral administration of Xugu Huoxue Decoction on the basis of control group.Both groups were given conventional treatment before and after operation.Compare the visual analog scale(VAS),hip joint function score,PINP,IL-6 value and fracture healing rate before treatment,1 month after treatment and 3 months after treatment,and compare the bone mineral density value,TCM syndrome score and complications before treatment and 3 months after treatment.Result:1.Patients in the observation group and control group were randomized in age,gender,fracture classification and other basic information,and there was no statistical significance in statistical test(P>0.05),indicating comparability.Preoperative hip function score(Harris),visual analogue score(VAS),biochemical indexes,bone density and TCM syndrome score were not statistically different between the two groups(P>0.05),which could be compared.2.The Harris scores of the observation group at different time points after surgery were higher than those before surgery,with statistically significant differences(P<0.05).The Harris score of the observation group was higher than that of the control group(P<0.05).3.VAS score: VAS score 1 month and 3 months after surgery in 2 groups was significantly lower than before surgery,and observation group was better tha n control group(P<0.05).4.Fracture healing rate: The fracture healing rate of the observation group was significantly better than that of the control group(P<0.05).5.Bone mineral density value: The bone mineral density(BMD)level of patients in both groups was higher than that before operation 3 months after operation,indicating that the two groups of treatment schemes had significant effect on osteoporosis(P<0.05).Compared with the control group,the bone mineral density of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).6.Biochemical indexes: The PINP and IL-6 levels in the observation group were significantly lower than those in the control group at 1 month and 3 months after operation(P<0.05);7.TCM syndrome score: The syndrome score of patients in the two groups after operation is lower than that before operation(P<0.01),It is proved that the two groups of treatment schemes have significant effects.The syndrome score in the observation group was better than that in the control group at 3 months after operation(P<0.05),and the difference was statistically significant.8.Complications: There were no complications such as coxa vara,lower limb venous thrombosis,internal fixation loosening and infection in the two groups during hospitalization and follow-up.Conclusion: PFNA internal fixation for elderly intertrochanteric fracture of femur has high certainty of efficacy,simple operation and low complication rate.Combined with Xingu Huoxue Decoction,PFNA internal fixation has more advantages than simple surgical treatment in visual pain simulation score,hip joint function score,biochemical indicators,fracture healing rate and bone density recovery.At the same time,integrated traditional Chinese and western medicine treatment has become a trend,With the increasing acceptance of patients,the clinical implementation is more convenient,so this scheme can be used for reference in clinical practice. |
