The Clinical Observation On The Treatment Of The Elderly Intertrochanteric Fracture With Xugu Huoxue Decoction And The PFNA Interual Fixation

Objective:By observing the Clinical observation on the treatment of the elderly intertrochanteric fracture with Xugu Huoxue decoction and the PFNA internal fixation,to accumulate the experience of traditional Chinese medicine and operation treating the elderly intertrochanteric fracture.Methods:56 patients who were 60-89 years old with intertrochanteric fractures were choosed as research object of this study.The study subjects were randomly divided into control group and treatment group.Both of the two groups patients were given conventional treatment preoperatively. Control group underwent PFNA operation treatment,and oral administration of Caltrate D 2 days after operation. Treatment group underwent PFNA operation treatment,and oral Xugu Huoxue decoction 2 days after operation.Two groups of patients with preoperative postoperative were given conventional treatment.Observe two groups of patients with postoperative pain visual analogue scale method(Visual analogue scales,VAS)score,fracture healing rate,hip Harris score and postoperative complications and other indexes of recording data.Results:1、Compared with the control group and the treatment group in postoperative first days,one week,two weeks,three weeks,the result of analysis of variance of repeated measurement data of VAS score results within 3 weeks after operation shows F=4.23,P<0.05,which displays that changes of VAS score was statistically significant. The curative effect of treatment group is more obvious than the control group in easing of postoperative pain of intertrochanteric femoral fracture.2、As the results of clinical healing rate,two groups of fracture healing rate was statistically significant both in 6 weeks after the operation(X2=4.37,P=0.04<0.05) and in 3 months after the operation(X2=4.08,P=0.04<0.05).So it is suggested that fracture healing rate both in 6 weeks after the operation and in 3 months after the operation were higher than that of control group.3、The results of two independent samples T test of hip function assessment situation in two groups show that differences of hip function assessment situation in two groups which dealed with treatment in 3 months(P=0.06>0.05)and 6 months(P=0.14>0.05)between the two groups are no statistically significant.It is not considered Harris score both of the treatment group and the control group 3 months and 6 months after operation are not the same.Hip function after operation of the two groups are satisfactory.4、The two groups after operation in the hospital and out hospital during the follow-up period did not appear coxa vara,bedsore,loosening of internal fixation of fracture,femoral neck and shaft fractures,deep vein thrombosis,pulmonary infection and other complications.Conclusion:Efficacy of PFNA internal fixation is better after adding oral Xugu Huoxue Decoction,which can promote the healing of the fracture,obviously improve the recovery of joint function and quality of life of patients after operation,and improve the patient’s physical condition by treatment based on syndrome in TCM theory,combined with the tongue and pulse theory,based on prescription.The advantage is complementary with the method of traditional Chinese medicine and Western medicine in this study,which is worthy of further promotion and application.
