| Objective: In this study,Bushen Tongqiao Ⅰ Decoction was used to treat patients with nervous tinnitus of kidney-yang deficiency type.By observing the changes of TCM symptom score,IHI score,tinnitus severity score,HAMA and HAMD score and sleep quality index before and after treatment,the effectiveness and safety of clinical medication of this method were evaluated,in order to find a treatment method with small adverse reactions and high titer,so as to improve the quality of life of patients,better play the advantages of traditional Chinese medicine,and provide a new thinking of traditional Chinese medicine for clinical treatment of nervous tinnitus of kidney-yang deficiency type.Methods: From January 2022 to December 2022,60 patients with nervous tinnitus of kidney yang deficiency type were randomly divided into two groups,30 in the treatment group and 30 in the control group.Both groups were treated with conventional western medicine(mecobalamin tablets,flunarizine hydrochloride capsules)for neurological tinnitus.The treatment group was treated with traditional Chinese medicine Bushen Tongqiao Ⅰ decoction on the basis of conventional western medicine treatment in the control group.After 4 weeks of treatment,the changes of various indicators before and after treatment were recorded,and all data were statistically analyzed using SPSS26.0statistical analysis software.Results:1.Comparability analysis of the two groups of patients : There was no significant difference in the general data and the indexes of each observation item before treatment(P > 0.05).2.Clinical efficacy evaluation : The total effective rate was 90 % in the treatment group and 76.6 % in the control group.The two groups were effective in the treatment of nervous tinnitus with kidney yang deficiency,but the clinical efficacy of the treatment group was significantly better than that of the control group,with statistical difference(P = 0.007 < 0.01).3.Evaluation of efficacy indicators :(1)TCM syndrome score :The comparison of the total scores of the two groups showed that the treatment group had better curative effect in improving the overall symptoms of traditional Chinese medicine,and the treatment group was better than the control group after treatment.Comparing the specific symptom scores of the two groups of traditional Chinese medicine,it was found that the two groups had better curative effects in improving tinnitus,soreness and weakness of waist and knees,dizziness,fatigue and fatigue or mental retardation.Comparison of cold limbs,lower limb edema,slippery sperm impotence or uterine cold infertility,the results showed that the treatment group had better curative effect,while the control group had poor curative effect.Comparison of frequent and clear long night urination,the results showed that the two groups of patients had no good effect in improving frequent and clear long night urination.(2)Tinnitus Handicap Inventory(THI)score : After comparison,the results showed that the two groups had better efficacy in improving the degree of tinnitus disability,and the treatment group was better than the control group.(3)Tinnitus severity score :By comparison,the results of the two groups in improving the overall severity of tinnitus have a good effect,but the treatment group than the control group.The specific circumstances of the score,by comparison,the results of the treatment group in improving the impact of tinnitus on sleep,tinnitus on life and work,the patient ’s overall feeling of tinnitus is better than the control group.The two groups were similar in improving the environment of tinnitus,the duration of tinnitus,and the effect of tinnitus on emotions.(4)Hamilton Anxiety Scale(HAMA)score : By comparison,the results showed that the two groups had better efficacy in improving anxiety symptoms,and the treatment group was better than the control group.(5)Hamilton Depression Scale(HAMD)score : By comparison,both groups had better efficacy in improving depressive symptoms,and the treatment group had more significant efficacy.(6)Pittsburgh sleep quality scale(PSQI)score : By comparison,both groups had better efficacy in improving sleep quality symptoms.The treatment group had better curative effect.(7)Safety indicators : Two groups of patients through the study found that there were no adverse reactions,indicating that the two groups of treatment methods are safe.Conclusion:(1)Bushen Tongqiao Ⅰ prescription combined with western medicine and western medicine alone in the treatment of renal yang deficiency type of nervous tinnitus have a good effect,improve the quality of life of patients.(2)The effect of Bushen Tongqiao Ⅰ combined with western medicine in improving tinnitus and accompanying symptoms of anxiety,depression and insomnia is significantly better than that of western medicine alone.(3)Bushen Tongqiao Ⅰ prescription pays attention to syndrome differentiation and treatment,improves the curative effect of nervous tinnitus with kidney yang deficiency,and has obvious advantages of traditional Chinese medicine,which is worthy of clinical application. |
