| Objective: Dynamic navigation-assisted jaw cyst fenestration decompression surgery has been carried out in model studies,but has not yet been applied clinically,and the real patient’s intraoral situation is more complex than the model.Therefore,in this study,dynamic navigation assist technology and traditional free-handed jaw cyst fenestration were used in clinical practice,and the accuracy,intraoperative situation and postoperative response of these two different surgical methods in jaw cyst fenestration were compared respectively.Therefore,the accuracy,minimally invasive and safety of dynamic navigation-assisted jaw cyst fenestration decompression are explored,and the application of navigation-assisted jaw cyst surgery is provided as a reference for clinical application.Methods: From January 2021 to December 2022,29 patients with jaw cysts were selected in the outpatient clinic of Wuzhou Red Cross Hospital.According to the three factors of gender,age,and cyst location,the patients who visited the patient were enrolled according to the distribution principle of the smallest imbalance index.It was divided into dynamic navigation group and the traditional surgery group,with 15 patients in the former and 14 patients in the latter.Both groups were preoperatively scanned by CT data acquisition,and the site and morphology of jaw cyst bone fenestration were designed in navigation-assisted surgery.During the operation,the dynamic navigation group guided the fenestration surgery through the navigator,and the traditional surgery group of the traditional group performed cyst fenestration according to the preoperative plan.After the completion of the operation,the postoperative CT of the patient was taken,the CT data was imported into professional software for processing,and the deviation of the fenestration area,the geometric center distance of the window,and the overlap rate of the fenestration area of the two groups of actual surgery and the preoperative design cyst bone were collected,and the data of the operation time,intraoperative blood loss and postoperative complications of the two groups were collected.SPSS21.0 was used to analyze the collected data of the two groups,and when the P<0.05 indicated that the difference between the two groups was statistically significant.Results: All patients in both groups successfully completed jaw cyst fenestration.After statistical comparative analysis of the average age,sex and cyst location of the general data of the two groups,the results showed P=0.321,P=0.710 and P=0.700,respectively,and the differences were not statistically significant.The surgical accuracy data of dynamic navigation group and traditional group were as follows: the center distance of the fenestration geometry was(1.09±0.20)mm and(2.70±0.45)mm,and the P<0.001 were statistically significant.The deviations of window area were(6.75±0.81)mm~2 and(7.45±1.02)mm~2,P=0.051,and the difference between the two groups was not statistically significant.The overlap rates of window area were(82.43±4.00)% and(50.74±6.04%)%,P<0.001,and the difference between the two groups was statistically significant.The operation time of the dynamic navigation group and the traditional surgery group was(30.06±2.23)min and(38.35±3.43)min,P<0.001,respectively,and the difference between the two groups was statistically significant.The intraoperative bleeding between the two groups was(9.33±2.36)ml and(11.31±3.46)ml,P=0.081,and the difference between the two groups was not statistically significant.Neither group experienced postoperative nerve numbness.The scores of the Visual analogue scale(VAS)in the Dynamic Navigation group were(3.02±0.58)and(2.11±0.49),respectively,and(4.04±0.83),(3.17±0.62),P <0.05 in the dynamic navigation group and the difference between the two groups was statistically significant.The pain rate in the dynamic navigation group on the second day after surgery was 6.7%,and 42.9% in the traditional surgery group,The pain rate in the Dynamic Navigation group on the second day after surgery was 6.7%,and in the traditional surgery group was 42.8%,P=0.035,and the difference between the two groups was statistically significant.Conclusion:(1)Compared with the traditional surgical jaw cyst fenestration,dynamic navigation technology assisted technology has higher accuracy.(2)Compared with the two groups,the dynamic navigation-assisted jaw cyst fenestration surgery took shorter time,the intraoperative blood loss was not obvious,the postoperative pain was milder,and the minimally invasive was better than that of the traditional surgery group.(3)There were no complications of postoperative nerve numbness in the two groups,and the safety of surgery was similar. |
