| Objective:To investigate the effect of fixed orthodontic treatment supplemented with different cycles of supracingival ultrasonic cleaning and supracingival ultrasonic cleaning combined with 25μm glycinine sand blasting on plaque control and enamel demineralization in patients. In order to provide a more effective method for fixed orthodontic patients to prevent periodontal disease and enamel demineralization and other orthodontic complications,to provide a more scientific reference for the clinical work of orthodontists. Methods:Patients with gingival inflammation or enamel deacidification within 6 months after first wearing fixed orthodontics in the Affiliated Stomatological Hospital of Jiujiang University from January 2022 to January 2023 were selected as subjects of this study, aged 10-25 years old.A double-blind self-controlled study was carried out,and the whole dentition was divided into left and right halves of the mouth. The control group was treated with supringival ultrasonic cleaning. The experimental group was treated with supracingival ultrasonic cleaning and supracingival sand blasting with 25 μm glycine sand powder. The subjects’ central incisor teeth, lateral incisor teeth, second premolar and first molar were used as Index teeth,a total of 480 teeth. The Sulcus Bleeding Index of the two groups was compared at the beginning of the experiment(baseline TO),1 month after the experiment(T1),and 4 months after the experiment(T2).SBI),Plaque index for orthodontic(PLI-O),enamel demineralization index.Results:1.Comparison between the experimental group and the control group at the baseline stage:there was little difference in the SBI value,PLI-O value and enamel demineralization index at the baseline stage of dentition between the two groups(P>0.05);2.Comparison within each time node in the experimental group:(1)SBI value:after the first month and the fourth month of the experiment,SBI value was lower than the baseline period, the first month of the experiment was lower than the fourth month of the experiment, the differences were statistically significant(p<0.001);(2)Enamel demineralization index:There was no significant difference between the baseline period, the first month and the fourth month(p>0.05);(3)PLI-O value:It was lower than the baseline level after the first month and the fourth month, and the PLI-O value in the first month was lower than the fourth month, with statistical significance(p<0.001).3.Intra-group comparison at each time node in the control group:(1)SBI value:after the first month and the fourth month of the experiment,SBI value was lower than the baseline period, the first month of the experiment was lower than the fourth month of the experiment, the differences were statistically significant(p<0.001);(2)Enamel demineralization index:the first month and the fourth month were higher than the baseline period, and the first month was lower than the fourth month, with statistical significance(p<0.05)(3)PLI-O value:After the first month and the fourth month of the experiment, the level of PLI-O in the first month was lower than that in the fourth month, and the differences were statistically significant(p<0.001).4.Comparison between the experimental group and the control group:with the extension of treatment time, the dental SBI and PLI-O values of the two groups showed a decreasing trend, and the values of the experimental group were significantly lower than the control group after the first month(T1)and the fourth month(T2),the difference was statistically significant(p<0.05).In general, the enamel demineralization index of the two groups showed an increasing trend with the passage of time, but the increasing trend of the experimental group was slower than that of the control group, and the difference between the two groups was statistically significant at T1 and T2 time points(p<0.05).Conclusion:1.Regular use of supragingival ultrasonic cleaning during fixed orthodontic treatment can effectively control plaque accumulation.2.During fixed orthodontic treatment, regular use of supragingival ultrasonic cleaning combined with 25 μm glycine sand blasting can more effectively control plaque accumulation and enamel demineralization compared with single use of supragingival ultrasonic cleaning. |
