| Objective:To investigate the changes of periodontal clinical indicators and the amount of gingival crevicular fluid in adolescents with self-ligating bracket appliance and conventional bracket appliance,and to compare the effects of the two fixed orthodontic appliance on periodontal status of adolescents.At the same time,the correlation between the amount of gingival crevicular fluid and periodontal clinical indicators was analyzed.Methods:60 adolescents aged 11-15 years were randomly selected for fixed orthodontic treatment in orthodontics department,and were divided into two groups according to the type of bracket devices.30 adolescents in the self-ligating group were treated with self-ligating bracket appliance,and 30 adolescents in the conventional group were treated with conventional bracket appliance.Periodontal clinical indicators(plaque index PLI,bleeding index BI,gingival index GI,probing depth PD,attachment loss AL)and the amount of gingival crevicular fluid were measured before fixed treatment and 1month,3 months,6 months after wearing appliances.Statistical methods:SPSS22.0 statistical software was used to analyze the measured data,and the measurement data was expressed as(_x±S).Repeated measurement analysis of variance was used to analyze the measured data at different times between the two groups,and P<0.05 was considered to be statistically different.The correlation between the amount of gingival crevicular fluid and periodontal clinical indicators was analyzed by Pearson correlation analysis.Results:1.PLI,BI,GI,PD and the amount of gingival crevice fluid in 2 groups after wearing fixed appliances were higher than before,the differences was statistically significant(P<0.001),and no periodontal attachment loss was observed during the observation period.2.The PLI of self-ligating group was lower than that of conventional group at the first,third and sixth months(P<0.05).The BI of self-ligating group was lower than that of conventional group at the sixth months(P<0.05).The GI of self-ligating group was lower than that of conventional group at the third and sixth months(P<0.05).There was no significant difference in PD between the two groups during observation period(P>0.05).The the amount of gingival crevicular fluid of self-ligating group was lower than that of conventional group at the first,third and sixth months(P<0.05).3.The amount of gingival crevicular fluid in the two groups was positively correlated with PLI,BI,GI and PD,among which the correlation with PLI was the most obvious(r=0.424).Conclusions:1.Both conventional and self-ligating bracket appliances can increase the periodontal clinical indicators and the amount of gingival crevicular fluid in adolescents at the initial stage of treatment,but do not cause attachment loss,shows that the effects of these two fixed orthodontic appliances on the periodontal tissues of adolescents were limited to causing gingivitis,but not periodontitis.2.The increase of PLI,BI,GI and the amount of gingival crevicular fluid in the self-ligating group was less than that in the conventional group,indicating that the self-ligating bracket appliance has more advantages than conventional bracket appliance in the periodontal maintenance of adolescent orthodontic patients.3.The amount of gingival crevicular fluid in adolescent patients with fixed orthodontic appliance was closely correlated with periodontal clinical indicators,and there was a significant positive correlation with PLI. |
