Objective:Compare the anterior scleral thickness(AST)of posterior scleral morphology in different types of pathological myopia,and explore the relationship between posterior scleral staphyloma and AST.Methods:In this cross-sectional study,a total of 56 of pathological myopia with type Ⅰ,Ⅱ,and Ⅲ posterior scleral staphyloma and an axial length(AL)of ≥ 28.0 mm who were treated at the Affiliated Eye Hospital of Nanchang University from March to December 2022 were collected for anterior optical coherence tomography(AS-OCT)ophthalmic imaging.B-scan scanning mode was used to scan four radial lines(upper,lower,temporal,and nasal),regard the scleral spur as a starting point,The thickness of the anterior sclera is calculated at an interval of 1 mm from the sclera spur to 5 mm of the four radial lines.Age: 20-50(mean age 37.2 ± 8.8)years old,eye axis(AL):28.03~33.04(mean AL 30.21 ± 1.54)mm,equivalent spherical lens(SE):-11.25~-26.75(mean SE-18.65 ± 4.64)D.There are 19 males and 37 females.According to the performance of AL and non mydriatic Oubao ultra wide angle laser scanning ophthalmoscope,divided them into five groups: Group Ⅰ 1(type Ⅰ wide macular type with 28 ≤ AL<30mm),Group Ⅱ 1(type Ⅱ narrow macular type with28 ≤ AL<30mm),Group Ⅲ 1(type Ⅲ peridisc type with 28 ≤ AL<30mm),GroupⅠ 2(type Ⅰ wide macular type with AL ≥ 30mm),and Group Ⅲ 2(type Ⅲ peridisc type with AL ≥ 30mm).Pearson correlation analysis,one-way ANOVA,and independent sample t-test were used to explore the correlation between posterior scleral staphyloma morphology,AL,and anterior scleral thickness.Results:There was a significant negative correlation between AL and AST along the four meridians(superior,inferior,temporal,nasal)(P<0.05).Age was positively correlated with the mean AST of the upper,temporal and nasal meridians(P < 0.05),but not with the inferior meridian(P>0.05).We did not seen significant correlation between gender,Km and the average AST of the superior,inferior,temporal,nasal meridians(P > 0.1).When AL were 28 ≤ AL<30 mm,the AST of type Ⅰ-wide macular type and type Ⅱ-narrow macular type was significantly different at the distance of 1mm from scleral spur to nasal meridian(P < 0.05).There was a significant differences in AST between type Ⅰ-wide macular type and type Ⅲ-perioptic disc type at the inferior meridian,the temporal meridian,and the distance between the temporal meridian and the scleral spur 2 mm(P<0.05).There was a significant difference in AST between type Ⅱ-narrow macular type and type Ⅲ-perioptic disc type at the scleral spur at the superior and temporal meridians(P<0.05).When AL≥30mm,there was a statistical differences between typeⅠ-wide macular type and type Ⅲ-perioptic type in the AST of scleral spur in the inferior and temporal meridians(P < 0.05).Conclusions:With the increase of the axis of the eye,the anterior scleral thickness in different types of posterior scleral staphylosis also varies.Myopia not only affects the posterior scleral thickness,but also changes the anterior scleral thickness accordingly.The anterior scleral thickness may become a predictor for monitoring the progression of myopia and the formation and classification of posterior scleral staphylosis. |
