Objective:To explore the effects of aromatherapy massage combined with guided imagery on improving dyspnea,anxiety,depression,sleep disorders and quality of life in patients with advanced lung cancer,and to provide effective non-pharmacological intervention measures and theoretical basis for the management of dyspnea symptoms in patients with advanced cancer.Method:This study was a quasi-experimental study.A total of 86 patients with advanced lung cancer who met the inclusion and exclusion criteria from the thoracic ward of a cancer hospital in Jiangxi Province from December 2021 to December 2022 were selected as objects of study,and were stochastically divided into control group and trial group in accordance with the table of random digits,with 43 cases in each group.The control group was given routine treatment and nursing in thoracic medicine,and the experimental group was given aromatherapy massage combined with guided imagery on the basis of the control group,and the intervention lasted for2 weeks.Basic data of patients were collected before intervention,and dyspnea(Borg)score,anxiety and depression(HADS)score,sleep disorder(PSQI)score and quality of life FACT-L(V4.0)score of patients in both groups were collected before intervention,at 1 and 2 weeks of intervention.Statistical analysis was performed using SPSS26.0 statistical software to evaluate the effect of aromatherapy massage combined with guided imagery on improving dyspnea and other related symptoms in patients with advanced lung cancer.Results:1.Baseline data: This study ultimately included 86 patients with advanced lung cancer,including 43 in the control group and 43 in the trial group.There was no statistical significance in the general information between the two groups(P>0.05).2.Comparison of dyspnea(Borg)scores: Prior to intervention,Borg score of dyspnea in the control group was(6.33±1.09),while in the trial group was(6.48±0.92),P=0.596,showing no statistical significance.After 1 week of intervention,Borg score of dyspnea in the control group was(6.56±0.98),while in the trial group was(5.64±1.01),in the trial group was lower than that of the control group,P<0.001,and the difference was statistically meaningful.After 2 weeks of intervention,Borg score of the control group was(6.79±1.01),the trial group was(4.07±1.05),Borg scale score of the trial group was sensibly under than that of the control group,P<0.001,the difference was statistically significant.Borg score of the two groups were analyzed by repeated measures of variance at different time points,and the results showed that the time effect and intergroup effect were statistically significant between the two groups,P<0.001,and the interaction effect between the two groups was significant(F=77.935,P<0.001).3.Comparison of anxiety and depression(HADS)scores: Prior to intervention,the aggregate points of HADS in the control group was(24.09±2.11),while in the trial group was(24.14±2.18),P=0.920,showing no statistical significance.The anxiety result of the control group was(12.93±1.93),and that of the trial group was(12.88±1.71),P=0.906,showing no statistical significance.The depression result of the control group was(11.16±1.75),and that of the trial group was(11.26±1.62),P=0.798,showing no statistical significance.After 1 week of intervention,the total HADS score of the control group was(24.07±2.30),and the total HADS score of the trial group was(19.70±2.21).The total HADS score of the trial group was lower than that of the control group,P<0.001,and the difference was statistically significant.The anxiety result of the control group was(12.88±1.80),while the anxiety result of the trial group was(10.53±1.82).The anxiety result of the trial group was less than that of the control group,P<0.001,and the difference was statistically significant.The depression score of the control group was(11.35±1.66),while the depression score of the trial group was(9.16±1.51),P<0.001,The depression score of the trial group was lower than that of the control group,and the difference was statistically significant.After 2 weeks of intervention,the total HADS score of the control group was(24.07±2.30),and the total HADS score of the trial group was(12.58±2.27).The aggregate points of HADS in the trial group was sensibly lower than that of the control group,P<0.001,and the difference was statistically significant.The anxiety point of the control group was(12.93±1.77),while the anxiety point of the trial group was(7.30±1.55).The anxiety point of the trial group was clearly lower than that of the control group,P<0.001,and the difference was statistically significant.The depression result of the control group was(11.14±1.52),while the depression of the trial group was(5.28±1.44).The depression score of the trial group was significantly lower than that of the control group,P<0.001,and the difference was statistically significant.Repeated measure analysis of variance was used to compare the total score of HADS and scores of anxiety and depression between the two groups at different time points.The results showed that the time effect and intergroup effect between the two groups were statistically significant,P < 0.001,and the interaction effect between the two groups was significant,P<0.001.4.Comparison of Sleep disorder(PSQI)score: Prior to intervention,PSQI score of the control group was(13.37±2.08),while the PSQI score of the trial group was(13.49±2.46),P=0.814,showing no statistical significance.After 1 week of intervention,PSQI score of control group was(13.12±1.89),the PSQI score of the trial group was(10.12±2.13),PSQI score of trial group was less than that of control group,P<0.001,the difference was statistically meaningful.After 2 weeks of intervention,the PSQI point of the control group was(13.58±2.02),while the PSQI score of the trial group was(5.88±2.04).The PSQI score of the trial group was markedly lower than that of the control group,P<0.001,and the difference was statistically significant.Repeated measure analysis of variance was used to compare the PSQI score between the two groups at different time points,and the results showed that the time effect and intergroup effect were statistically significant between the two groups,P<0.001,and the interaction effect between the two groups was significant(F=57.525,P<0.001).5.Comparison of quality of life FACT-L(V4.0)score: Prior to intervention,FACT-L(V4.0)score of control group was(70.00±10.25),and the trial group was(69.53±9.08),P=0.824,showing no statistical significance.After 1 week of intervention,the FACT-L(V4.0)result of the control group was(69.65±9.66),while the trial group was(76.33±10.61),and the FACT-L(V4.0)score of the control group was higher than that of the trial group,P<0.001,the difference was statistically significant.After 2 weeks of intervention,the FACT-L(V4.0)score of the control group was(70.91±9.82),and the FACT-L(V4.0)score of the trial group was(90.33±10.42),and the FACT-L(V4.0)score of the trial group was markedly higher than that of the control group,P<0.001,the difference was statistically meaningful.The quality of life FACT-L(V4.0)scores of the two groups were analyzed by repeated measures of variance at different time points,and the results showed that the time effect and intergroup effect were statistically significant between the two groups,P<0.001,and the interaction effect between the two groups was significant(F=28.055,P<0.001).Conclusion:Aromatherapy massage combined with guided imagery can effectively alleviate the subjective feelings of dyspnea in patients with advanced lung cancer,reduce negative emotions of anxiety and depression,improve the quality of sleep and life of patients,providing a theoretical basis for the non-pharmacological treatment of cancer patients with dyspnea,and has clinical application value.
