Objective: Considering that there may be differences in the risk factors of colorectal adenomatous polyps in different regions and different populations,this study further explored the risk factors related to colorectal adenomatous polyps in some populations in Xi ’an area and the correlation between colorectal adenoma and Helicobacter pylori.Research object and method:Among the inpatients who underwent electronic colonoscopy and endoscopic pathological biopsy in Xi ’an Central Hospital from January2019 to September 2022,544 patients in the colorectal adenoma group were selected according to the inclusion criteria and exclusion criteria.In addition,650 hospitalized patients who underwent colonoscopy during the same period without abnormalities were used as control group.General information was collected,including gender,age,past history(hypertension,diabetes,coronary heart disease,gallbladder),medication history(aspirin),personal history(smoking,drinking),and family history(first degree relatives with CRC).The auxiliary examination results of the patients were collected,including UA,TC,TG,LDL-C,fasting 14 C or 13C-urea breath test,abdominal ultrasound or abdominal CT examination to confirm the presence of fatty liver,gallstone and gallbladder polyp,and gastroscopy to confirm the presence of gastric polyp.The number,size,location,morphology and surface mucosa of adenomas in the adenoma group were recorded according to the results of colonoscopy report,and the pathological classification and atypia of adenomas were recorded according to the results of pathological report.At the same time,the data of patients who underwent colonoscopy and found no abnormalities were recorded(the same as above,except the results of colonoscopy and pathology report).Comparisons were made between the two groups.Result:The incidence of colorectal adenomatous polyps of different genders increased with age,and the peak of colorectal adenomatous polyps was ≥45 years old.In the comparison between the adenoma group and the normal group,there were statistical differences in males,old age,hypertension,coronary heart disease,smoking,gallbladder disease,gastric polyps and UA(P<0.05).Multivariate logistic regression analysis was performed for statistically significant variables,and the results showed that females(OR: 0.426,95%CI: 0.331-0.547,P=0.000),age 45-59 years(OR: 2.773,95%CI:1.871-4.108,P=0.000),age ≥60(OR: 5.310,95%CI: 3.553-7.937,P=0.000),gastric polyps(OR: 1.529,95%CI: 1.159-2.018,P=0.003).Conclusion: 1.The incidence of colorectal adenomatous polyps of different genders will increase with age,and the peak of colorectal adenomatous polyps is ≥45 years old.2.Male,advanced age,hypertension,coronary heart disease,smoking,gallbladder disease,gastric polyp,and UA are risk factors for colorectal adenomatous polyps,and male,advanced age,and gastric polyp are independent risk factors for colorectal adenoma.Helicobacter pylori infection is not associated with colorectal adenoma and is not recommended as an indication for colonoscopy screening.3.Colorectal adenomas with diameter <1cm,number <3,located in the left colon,smooth surface mucosa,and no sessile adenomas were more common.The most common pathological types were tubular adenoma and low grade intraepithelial neoplasia.
