Objective:To explore the relationship between Helicobacter pylori(Hp)and colorectal polyps occurrence,recurrence and carcinogenesis by studying the Hp infection rate in patients with colorectal polyps and colorectal cancers,and by detecting the colorectal polyps recurrence after Hp eradication therapy,to find new way to prevent the colorectal polyps occurrence,recurrence and carcinogenesisMethods:1.1670 patients who underwent colonoscopy and Hp test in our hospital from January 2015 to December 2016 were collected and compared Hp infection rate in patients with colorectal polyps group,colorectal cancer group and control group(colonoscopy is normal).2.A total of 210 colorectal polyps patients underwent 13C-UBT examination and endoscopic resection,and follow-up colonoscopy at 1 years.During the follow-up,8patients were lost,202 patients were included in this study finally.According to Hp test results divided into Hp-positive(n=88)and Hp-negative(n=114)two groups,Hp-positive patients were randomly divided into eradication group(n=57)and non-eradication group(n=31),the recurrence rate of polyps between Hp-positive group and Hp-negative group,Hp-positive eradication group and non-eradication group were compared.Result:1.Hp infection rate in polyps group was higher than control group,the difference was statistically significant(P<0.05),Hp infection rate in tubular adenoma polyps group,tubular villous adenoma polyps group,low grade intraepithelial neoplasia group and colorectal cancer group was higher than control group,the difference was statistically significant(P<0.05),but the difference was not statistically significant in hyperplastic polyps group and high grade intraepithelial neoplasia group(P>0.05).Based on the sex,the infection rate of Hp in women with tubular adenoma polyps group,tubular villous adenoma polyps group was higher than that in women control group,the difference was statistically significant(P<0.05),but the difference was not statistically significant in hyperplastic polyps group for women(P>0.05),and the difference was not statistically significant among in hyperplastic,tubular adenoma polyps and tubular villous adenoma polyps for male(P>0.05).Both in single polyp group and multiple polyps group,the infection rate of Hp was higher than control group(P<0.05),the infection rate of Hp in polyps with diameter less than 1cm and diameter greater than or equal to 1cm was higher than control group(P<0.05).2.The recurrence rate of polyps in Hp-positive group was higher than that in Hp-negative group,and the recurrence rate of polyps in eradication group was lower than that in non-eradication group(P<0.05).Hp infection,male and multiple polyp were risk factors for the recurrence of colorectal polyps(P<0.05),age,pathology types of polyp,intraepithelial neoplasia and polyp size were not risk factors for the recurrence of colorectal polyps(P>0.05).Conclusion:1.Hp infection is associated with the occurrence and carcinogenesis of colorectal polyps;2.Hp infection significantly increases the risk of female adenomatous polyps;3.Hp infection is closely related to the recurrence of colorectal polyps,the eradication of Hp treatment can reduce the recurrence of colorectal polyps. |
