Objective: To investigate the risk factors of colorectal cancer and colorectal adenoma,and to analyze the differences of risk factors in different parts of colorectal cancer,to guide early clinical prevention and screening work.Methods: A retrospective analysis of the data of inpatients who underwent colonoscopy at the Second Hospital of Hebei Medical University from January 2014 to June 2020,included 1,250 patients who were pathologically confirmed as colorectal cancer into the colorectal cancer group,and the pathological diagnosis was nodal.1487 patients with rectal polyps were included in the polyp group,and 1,800 patients whose examination results showed no abnormalities served as the control group.The age,gender,smoking history,drinking history,bowel habits,cholesterol and triglyceride levels of the enrolled persons were collected by a combination of hospitalization medical record information system and telephone follow-up.Using SPSS26.0 for multivariate Logistic regression analysis,the odds ratio(OR value)and 95% confidence interval(95% CI)of each risk factor were calculated,and P <0.05 was considered statistically significant.Results: 1.Risk factors for colorectal cancer(1)Age,gender,smoking history,drinking history,chronic diarrhea,chronic constipation,and cholesterol levels were significantly different between the CRC group and the control group(P<0.05).There was no statistical difference in other indicators(P>0.05).(2)After the 1:1 match between the groups,the conditional logistic regression analysis still showed that chronic diarrhea was significantly different between the two groups(P<0.05).2.Risk factors for left and right colon cancer and rectal cancer(1)Left colon cancer: Age,drinking history,chronic diarrhea and cholesterol levels are significantly correlated with the occurrence of left colon cancer(P<0.05).There was no statistical difference in other indicators(P>0.05);(2)Right colon cancer: Gender,age,history of drinking,chronic diarrhea,chronic constipation,and the occurrence of right colon cancer were all significantly correlated(P<0.05).There was no statistical difference in other indicators(P>0.05);(3)Right colon cancer:Gender,age,history of smoking and drinking,chronic diarrhea,cholesterol levels and rectal cancer are all significantly correlated(P<0.05).There was no statistical difference in other indicators(P>0.05).3.Risk factors for colorectal polyps(1)Gender,age,smoking history,drinking history,chronic diarrhea,chronic constipation,cholesterol and triglyceride levels were significantly different between the polyp group and the control group(P<0.05).There was no statistical difference in other indicators(P>0.05);(2)After 1:1 matching between groups,the results of conditional logistic regression analysis still showed that chronic diarrhea,chronic constipation and triglyceride levels were significantly correlated with the occurrence of colorectal polyps(P<0.05).4.Risk factors for different types of colorectal polyps:(1)Non-adenomatous polyps: gender,age,chronic diarrhea,chronic constipation,alternating diarrhea and constipation,cholesterol,triglyceride levels and colorectal non-adenomatous polyps There was a significant correlation between the two groups(P<0.05).There was no statistical difference in other indicators(P>0.05);(2)Adenomatous polyps: gender,age,drinking history,chronic diarrhea,chronic constipation,cholesterol and triglyceride levels are significantly correlated with the occurrence of colorectal adenomatous polyps(P<0.05).There was no statistical difference in other indicators(P>0.05).Conclusions:1.Elder,male,smoking,drinking,chronic diarrhea,chronic constipation,and elevated cholesterol levels are independent risk factors for colorectal cancer.2.Elevated cholesterol level is an independent risk factor for left colon cancer and rectal cancer;male is an independent risk factor for right colon cancer and rectal cancer;chronic constipation is an independent risk factor for right colon cancer;smoking history is rectal cancer independent risk factors for cancer.3.Male,advanced age,history of smoking and drinking,chronic diarrhea,chronic constipation,elevated triglycerides and cholesterol levels are risk factors for colorectal polyps.The above risk factors are independent risk factors for the occurrence of adenomatous polyps except smoking history;the above risk factors are independent risk factors for the occurrence of non-adenomatous polyps except for the history of smoking and drinking.The alternation of diarrhea and constipation is also the risk factor. |
