| Study 1.To explore the difference in the severity of dysarthria between left and right cerebral strokeObjective: Based on the mandibular oral motor assessment scale,the motor functions of lips,tongue and mandibular arthritic organs in patients with unilateral upper motor neuronal dysarthria after stroke were evaluated,and the severity of dysarthria on the left and right sides of stroke was compared,to explore whether there is a left advantage in dysarthria.Methods: The data of 27 patients with dysarthria in left stroke and 25 patients with dysarthria in right stroke were collected.The mandibular motor function evaluation scale was used to evaluate all subjects,including the comprehensive evaluation of mandible,lip and tongue,and each sub-item was graded by a 0-4 severity grade.finally,the total score was calculated and the total severity score was obtained.the two-sample t-test was used to compare the two groups,and the one-way ANOVA was used in the group.Results:1.Comparison results between groups: There was a statistically significant difference in the mean value of the mandibular-oral motor function score between the left-side stroke group and the right-side stroke group(p<0.01);2.Intra-group comparison results: There was no statistically significant difference in the mean value of the mandibular-oral motor function scores in different lesions(basal ganglia,pons,corona radiata and center of semiovale)of unilateral stroke(p>0.05).Conclusions:1.Left cerebral lesions showed more severe dyskinesias of the tongue and face,dysarthria has dominant in the left cerebral hemisphere after stroke.2.There was no difference in the severity of dysarthria among different lesion sites(basal ganglia,pons,corona radiata and center of semiovale)in unilateral stroke.Study 2.To explore the effectiveness of vowel quantitative acoustic index in the evaluation of dysarthria in UUMN after stroke.Objective: Collect speech samples from patients with post-stroke UUMN dysarthria and normal adults for a comparative study of F1,F2,VSA,VAI,FCR and other acoustic parameters to explore the effectiveness of vowel quantitative acoustic indexes in evaluating post-stroke UUMN dysarthria.Methods: The formants F1 and F2 of vowels in UUMN dysarthria group(52 cases)and normal adult group(38 cases)were extracted and brought into the formula to calculate the mean values of VSA,VAI and FCR acoustic parameters.The data obtained were compared by two-sample t-test.Results: Compared with the normal control group,/a/F2,/ i/F1,/u/F2 were significantly increased and /a/F1 were significantly decreased in UUMN dysarthria group(P<0.01),but the mean value of /u/F1 had no significant difference.There were significant differences in the mean values of VSA,VAI and FCR between UUMN dysarthria group and normal adult group(p>0.05).Conclusions: Vowel quantitative acoustic parameters VSA,VAI and FCR can effectively distinguish normal adults from UUMN dysarthria patients,and are reliable indicators for evaluating dysarthria.Study 3.To explore the left side dominance of UUMN dysarthria after stroke based on vowel quantitative acoustic index.Objective: To compare the differences of F1,F2,VSA,VAI,FCR and other acoustic parameters of vowel speech samples of left and right UUMN dysarthria patients,and to explore whether UUMN dysarthria has a left side dominance after stroke.Methods: The vowel formants F1 and F2 of 27 patients with dysarthria in left stroke and 25 patients with dysarthria in right stroke were extracted and substituted into the formula to calculate the values of VSA,VAI and FCR.The data obtained were compared between groups by two-sample t-test and within groups by single-factor analysis of variance.Results:1.Comparison results between groups: There were significant differences in the average values of /i/F1,/ i/F2 and / u/F2 between the left stroke group and the right stroke group(p<0.01).However,there was no significant difference in the mean values of / a/F1,/ a/F2 and / u/F1(p>0.05).There were significant differences in the mean values of VSA,VAI and FCR between the left stroke group and the right stroke group(p<0.01).2.Intra-group comparison results: There was no significant difference in the mean values of VSA,FCR and VAI of dysarthria caused by different lesions(basal ganglia,pons,corona radiata and center of semiovale)in unilateral stroke(p>0.05).Conclusion: Compared with the right stroke group,FCR in the left stroke group increased significantly,VSA and VAI decreased significantly.UUMN dysarthria after stroke has the advantage of left cerebral hemisphere. |
