ObjectiveBased on the Information-Motivation-Behavioral skills model(IMB skill model),the nursing intervention program was constructed for the social participation level of middle-aged and young stroke patients,and the effects of the intervention program were verified through quasi-experimental studies.Methods1.Construction of intervention scheme based on IMB skill model for the social participation level of middle-aged and young stroke patients.The present situation and influencing factors of the social participation level of young and middle-aged stroke patients were investigated by using the cross-sectional survey method.According to the results of the cross-sectional survey and under the guidance of the THEORY of IMB skill model,the preliminary intervention plan was formed.Further,7 experts were selected to consult the first draft of the intervention plan,and the intervention content was improved according to the expert opinions.Then,8 patients were selected for the pre-experiment,and the intervention plan was further improved according to the results of the pre-experiment,and finally the final draft of the intervention plan was formed.2.Verify the effect of nursing intervention program based on IMB skill model on the social participation level of young and middle-aged stroke patients.From April to November 2021,79 patients with cerebral apoplexy who met the inclusion criteria in a class A general hospital in Luoyang city were selected as the research objects for quasi-experimental study.Routine nursing intervention of the control group was given priority.After the intervention of the control group,the intervention group was based on the intervention plan of the control group.A comprehensive nursing intervention program based on IMB skill model was adopted.Data were collected on the first day of admission,the day of discharge,the first month and the third month after discharge,and the social participation level,stroke health knowledge level,neurological function recovery,anxiety and depression and other indicators of patients in the two groups were observed.Result1.Construction of nursing intervention program of social participation level for young and middle-aged stroke patients based on IMB skill modelThe results showed that 306 young and middle-aged stroke patients were 24-59 years old,with an average of(50.23±6.82)years old.The score of autonomic participation questionnaire was(31.70±15.74),and the social participation level needed to be improved.The factors affecting the social participation level of young and middle-aged stroke patients were neurological recovery,disability,daily living ability,educational level and economic level(P < 0.05).Under the theoretical guidance of IMB skill model,the preliminary draft of the intervention program was constructed based on the current situation investigation,and further revised the preliminary draft of the intervention program through expert discussion and preexperiment,and finally formed the final draft of the intervention program.For details,see Table 2-9.2.Application effect of IMB skill model on nursing intervention program of social participation level in young and middle-aged stroke patients(1)Health knowledge level of young and middle-aged stroke patients improved after intervention: the scores of stroke health knowledge at each time point after intervention increased gradually over time,and the intervention group was superior to the control group,with statistical difference(P < 0.05).Repeated measurement variance analysis of stroke health knowledge score at each time point of the two groups showed that the health knowledge level of the two groups had a temporal effect(F = 740.633,P < 0.05),inter-group effect(F = 26.624,P < 0.05)and interaction effect between the two groups(F = 4.798,P < 0.05).(2)After intervention,the social participation level of young and middle-aged stroke patients was improved: each time point in the two groups after intervention in chinese patients with ipa score over time to reduce gradually,social participation level of intervention group is better than that of control group,with statistical significance(P < 0.05),the two groups of patients with independent participation questionnaire scores at each time point for repeated measurements analysis of variance,two groups of chinese independent participation questionnaire scores in the time effect(F = 183.334,P < 0.05),inter-group effect(F = 183.334,P < 0.05)and interaction effect between the two groups(F = 6.453,P < 0.05).(3)Anxiety scores of young and middle-aged stroke patients after intervention:the scores of anxiety at each time point in the intervention group decreased over time,and the intervention group was better than the control group,with statistical significance(P < 0.05).Repeated measure anova showed that the scores of anxiety in the two groups had significant temporal effect(F = 597.190,P < 0.05),intergroup effect(f = 128.345,p < 0.05)and the interaction between the two groups(F =209.369,P < 0.05).(4)Depression scores of young and middle-aged stroke patients decreased after intervention: after intervention,the depression scores at each time point in the two groups decreased with time,and the depression scores at each time point in the intervention group were lower than those in the control group,which was statistically significant(P < 0.05).repeated measure anova showed that the depression scores in the two groups had significant temporal effect(F = 174.584,P < 0.05),inter-group effect(f = 196.007,p < 0.05)and the interaction between the two groups(F =210.125,P < 0.05).(5)The neurological function of young and middle-aged stroke patients improved after intervention: the nhiss scores of patients in the two groups at each time point of intervention decreased with the passage of time,and the neurological function recovery of the intervention group was better,with statistical significance between the two groups(P < 0.05).repeated measure anova showed that the nhiss scores of patients in the two groups had a time effect(F = 642.472,P < 0.05),the inter-group effect(F = 147.302,P < 0.05)and the interaction effect between the two groups(F = 194.772,P < 0.05)had statistical significance.(6)The disability of young and middle-aged stroke patients improved after intervention: after intervention,mrs scores of the two groups at each time point decreased over time,and the degree of disability in the intervention group was lighter than that in the control group.there was statistical significance between the two groups(P < 0.05).repeated measure anova showed that mrs scores of the two groups had significant temporal effect(F = 692.564,P < 0.05),inter-group effect(f = 35.508,p < 0.05)and the interaction between the two groups(F = 122.350,P < 0.05).(7)The quality of life of young and middle-aged stroke patients was improved after intervention: after the intervention,the pap index scores of the two groups at each time point increased with the passage of time,and the daily living ability of the intervention group was stronger than that of the control group,and there was statistical significance between the two groups(P < 0.05).the results of repeated measure anova showed that the pap index scores of the two groups had a significant time effect(F = 234.774,P < 0.05),inter-group effect(F = 499.854,P < 0.05)and interaction effect between the two groups(F = 608.123,P < 0.05).ConclusionThe level of social participation of young and middle-aged stroke patients needs to be improved;The level of education,economic level,neurological recovery,disability and ability of daily life are the important factors affecting the level of social participation.Nursing intervention program based on IMB skill model can improve the social participation level,health knowledge level and life quality of young and middle-aged stroke patients.Reduce the occurrence of anxiety and depression after stroke;Accelerate the recovery of patients’ neurological function;Reduce the incidence of disability in patients.
