Objectives1.Construct an intervention program for the sense of benefit of young and middleaged stroke caregivers based on positive self-disclosure;2.To explore the application effect of the intervention program on improving the sense of benefit,care burden and quality of life of young and middle-aged stroke caregivers.MethodsThis study consists of two parts.1.To construct an intervention program based on positive self-disclosure of benefit finding for caregivers of young and middle-aged stroke patients:based on the method of literature analysis,summarized and analyzed the current situation of the intervention of sense of benefit of caregivers and self-disclosure of caregivers at home and abroad,and conducted qualitative interviews with 10 young and middle-aged stroke caregivers to construct the first draft of the intervention program.Further two rounds of Delphi expert consultation were conducted,and six young and middle-aged stroke caregivers were selected for pre-test.The intervention plan was revised and improved to form the test draft of the intervention plan.2.Application and effect evaluation of the intervention program for the sense of benefit of young and middle-aged stroke caregivers based on positive self-disclosure:young and middle-aged stroke caregivers who met the inclusion and exclusion criteria in the two wards of the Department of Neurology of a class III class a hospital in Zhengzhou were randomly divided into control group and intervention group,with 39 cases in each ward.The control group implemented routine health education in neurology,and the intervention group implemented the benefit intervention program for young and middle-aged stroke caregivers based on positive self-disclosure on the basis of the control group.Before intervention,immediately after intervention,one month after intervention and three months after intervention,stroke caregiver benefit questionnaire,Zarit Caregiver Burden Scale and adult caregiver quality of life questionnaire were used to collect data to evaluate the intervention effect.SPSS 26.0 software was used to record and analyze the data,and t-test,rank sum test,repeated measurement analysis of variance The generalized estimation equation were used for statistical analysis,and the change trend of each group of variables with time was analyzed by graphpad prism software.Results1.Based on positive self disclosure,the intervention program of middle-aged and young stroke caregivers’ sense of benefit lasted for 4 weeks,twice a week,a total of 8 times.The themes of the intervention were:building relationships and enhancing the sense of control;Know and show,enhance the sense of control;Establish positive emotions and find meaning;Maintain positive emotions and self-improvement;Establish positive behavior and self enhancement;Maintain positive emotions and explore meaning;Peer support,exploring meaning;Review the process and look forward to the future.The first four times were face-to-face intervention and the last four times were wechat intervention.2.In this study,3 cases(8.3%)were lost in the control group and the experimental group,and the effective sample size were 36 cases in each group.The average age of the intervention group was(51.36±6.45)years,and that of the control group was(52.08±5.45)years.(1)The level of sense of benefit of young and middle-aged stroke caregivers after the intervention based on positive self disclosure was higher than that of routine health education(F=54.172,P<0.001).There was a difference in the score of sense of benefit between the intervention group and the control group at each time point(F=864.805,P<0.001),and there was an interactive effect between time and different treatment factors(F=598.631,P<0.001).(2)After the intervention based on positive self disclosure,the care burden of young and middle-aged stroke caregivers reduced more than that of routine health education(F=371.046,P<0.001).There were differences in the scores of care burden between the intervention group and the control group at each time point(F=358.874,P<0.001),and there were interactive effects between time and different treatment factors(F=148.029,P<0.001).(3)The improvement of life quality of young and middle-aged stroke caregivers after the intervention based on positive self disclosure was higher than that of routine health education(F=236.018,P<0.001).The scores of life quality in the intervention group and the control group were different at each time point(F=268.501,P<0.001),and there was an interactive effect between time and different treatment factors(F=152.034,P<0.001).(4)There were significant differences in the scores of caregivers’ sense of benefit and caregivers’ quality of life between the intervention group and the control group immediately after the intervention,one month and three months after the intervention(P<0.05),but there was no significant difference three months after the intervention;There was a significant difference in caregiver burden scores immediately after the intervention and one month after the intervention(P<0.001),but there was no significant difference three months after the intervention(P>0.05).Conclusions1.Based on positive self-disclosure,the intervention program of middle-aged and young stroke caregivers’ sense of benefit can improve caregivers’ sense of benefit,and reduce caregivers’ burden and improve their quality of life.2.The intervention program for the sense of benefit of young and middle-aged stroke caregivers based on positive self-disclosure can improve the mental health of caregivers and provide a valuable reference for the positive psychological intervention of young and middle-aged stroke caregivers in hospitals.
