Objectives To determine the levels of body composition,blood lipids and serum leptin in patients with Gestational diabetes mellitus(GDM)from 24-28 weeks of gestation,and to explore the relationship between body composition,glycolipid metabolism and serum leptin.Methods According to the diagnostic criteria of GDM recommended by IADPSG,pregnant women who underwent 75 g Oral glucose tolerance test(OGTT)in the Obstetrics department of Hebei general Hospital from April 2021 to July 2021 were divided into two groups according to the results of OGTT: namely the GDM group(61cases)and Normal glucose tolerance(NGT)group(75 cases).The level of serum leptin was detected by Enzyme-linked immunosorbent assay(ELISA),and the value of blood lipid was recorded.The datas of body composition analysis were taken,and their newborn birth weight and pregnancy outcome were followed up.Results 1 The Fat mass(FM)and Fat mass percentage(FMP)in GDM group were higher than those in NGT group from 24-28 weeks of gestation.The levels of FINS(Fasting insulin),HOMA-IR(Homeostasis model assessment insulin resistance),and TG/HDL-C(Triglyceride to high density lipoprotein cholesterol ratio)in GDM group were higher than those in NGT group,while serum Total cholesterol(TC)and High density lipoprotein cholesterol(HDL-C)were lower than those in NGT group(P<0.05).There was no significant difference in leptin level between the two groups(P>0.05).The total weight gain of GDM group was lower than that of NGT group,and the difference was statistically significant.2 Logistic regression analysis showed that FMP was an independent risk factor for GDM.3 The area under the curve(0.635)when using FMP to predict GDM was similar to that predicted by FMI.4 The correlation analysis between body composition and blood glucose at each time point of OGTT,blood lipid and other indexes showed that FMP,FM,FMI were positively correlated with OGTT at each time point,while muscle-to-fat ratio was negatively correlated with each time point value.TG was positively correlated with FMP,FM and FMI.TG was negatively correlated with Protein,muscle-to-fat ratio and residual capacity.HDL-C was negatively correlated with FMP,FM and FMI,while HDL-C was positively correlated with muscle-to-fat ratio and residual capacity.TG/HDL-C was positively correlated with FMP,FM and FMI,while TG/HDL-C was negatively correlated with Protein,muscle-to-fat ratio and residual capacity.There was no significant correlation between serum Leptin and body composition.5 There was a positive correlation between newborn birth weight and body composition.Conclusions 1 FMP in the second trimester of pregnancy is an independent risk factor for GDM.2 For the pregnant women diagnosed with GDM in the second trimester of pregnancy,the guidance of body composition analysis,diet education and exercise management and control can effectively ensure the increase of their total weight during pregnancy and the birth weight of newborns in a reasonable range.3 In the analysis of body composition of pregnant women,FMP was positively correlated with FPG and TG.4 TG was positively correlated with FMP,FM and FMI.TG was negatively correlated with Protein,muscle-to-fat ratio and residual capacity.HDL-C was negatively correlated with FMP,FM and FMI,while HDL-C was positively correlated with muscle-to-fat ratio and residual capacity.5 There was no significant correlation between body composition analysis and serum Leptin level.Figure: 1;Table:11;Reference:158...
