| Objective: To observe the clinical effect of Qizhi Decoction on edema,pain,exudation,granulation growth,and wound healing time after anal fistula surgery,and to provide clinical information for the clinical application and development of Qizhi Decoction in the treatment of wound healing after anal fistula surgery.in accordance with.Methods: From January 2022 to February 2023,60 patients who were diagnosed and treated in the Department of Gastrointestinal and Anal Surgery of the First Affiliated Hospital of Guangxi University of Traditional Chinese Medicine and met the diagnostic criteria of high-level simple anal fistula of damp-heat injection type and underwent surgical treatment were selected as treatment objects.The surgical method was incision and drainage of anal fistula.The 60 patients included in the study will be allocated according to the principle of random distribution,30 of them will be divided into the treatment group(sitz bath with Qizhi soup),and the other 30 patients will be divided into the control group(sitz bath with potassium permanganate);From the first day after the operation,the patients in the two groups underwent routine wound dressing changes in the morning and afternoon,respectively,and the patients in the study group were treated with fumigation and sitz bath with Qizhi Decoction,an anorectal washing medicine taken from "Surgery Zhengzong".The control group was treated with 1/5000 potassium permanganate solution for fumigation and sitz bath,and the wound pain,edema,exudation and wound pain on the 1st,3rd,7th,14 th,and 21 st days after operation were recorded respectively.healing time.The collected data were statistically analyzed using SPSS22.0 statistical software.Results: In the clinical experimental study,60 cases were observed in the experiment,30 cases in the treatment group and 30 cases in the control group.There was no significant difference in age,gender,and original wound area between the two groups of patients(P>0.05),and it is feasible to compare the data between the two groups.(1)In terms of wound pain: there was no significant difference in the pain scores between the two groups on the first day after surgery(P>0.05),and the data of the two groups were comparable.There was a statistically significant difference in the wound pain scores on the seventh day after surgery(P<0.05),on the 3rd,14 th,and 21 st days,there was no significant difference in pain scores between the two groups(P>0.05),suggesting that Qizhi Decoction had a better effect on the 7th day than potassium permanganate solution in terms of pain relief.Although there was no statistical significance in the comparison of pain scores on the 3rd,14 th,and 21 st days after the medication,if there was no difference,combined with the scores,the improvement trend of Qizhi Decoction was more obvious than that of the potassium manganate group.(2)In terms of wound exudation: the exudation score on the first day after operation was not statistically significant(P>0.05),the data of the two groups were comparable,although the exudation score of the two groups on the third day after treatment was not statistically significant(P>0.05),but there was a tendency to improve the exudation,the exudation was statistically significant on the 7th day after operation(P<0.01),and the exudation was statistically significant on the 14 th and21st day after operation(P<0.05),indicating that Compared with potassium permanganate fumigation and sitz bath,Qizhi decoction fumigation and sitz bath can more effectively reduce wound exudation at 7,14,and 21 days after operation.(3)In terms of wound edema: there was no significant difference in the edema score on the first day after operation(P>0.05),and the data of the two groups were comparable.Both showed a tendency to improve the exudation,indicating that the two groups had similar curative effects on edema on the 3rd and 21 st days after treatment.The comparison of edema scores on the 7th and 14 th day of medication was statistically significant(P<0.05),indicating that in the early and mid-term recovery from surgery,sitz bath with Qizhi Decoction is better than potassium permanganate fumigation sitz bath in relieving wound edema.(4)In terms of granulation growth: there was no significant difference in the granulation scores on the first day after operation(P>0.05),and the data of the two groups were comparable.On the third,seventh,14 th,and 21 st days,there was no significant difference in the granulation scores between the two groups(P>0.05).indicating that the Qizhi Decoction may have a certain promoting effect on granulation growth.(5)Healing time: By comparison,the difference in healing time between the two groups was statistically significant(P<0.05),which indicated that Qizhitang fumigation and washing sitz bath was better than potassium permanganate fumigation and washing sitz bath in shortening the healing time.(6)Overall curative effect: The effective rate of the treatment group and the control group after treatment was 83.33% in the control group and 86.67% in the treatment group,with no statistically significant difference(P>0.05).Both groups had the effect of improving the symptoms of exudation,pain and edema,promoting the growth of granulation and reducing the time of wound healing.The two groups had similar effects in pain,edema and granulation growth,but the data showed that Qizhi decoction was slightly better than potassium permanganate group.In terms of shortening healing time and reducing exudation,Qizhi decoction was significantly better than potassium permanganate group.Conclusion: Qizhi decoction sitz bath can relieve postoperative wound pain,exudation and edema of damp-heat pouring anal fistula and low-position simple anal fistula,accelerate the growth of granulation,and shorten the wound healing time.Moreover,the clinical safety is good,and there is no adverse reaction,so it is worthy of popularization. |
