Objectives:All kinds of major emergencies may occur at anytime and anywhere and often cause a large number of casualties.Henan,a province with a large population and convenient transportation,is in the central and western regions.The economy society is in a critical period of high-quality transformation and development,and there is a higher demand for public health security.So improving and optimizing the emergency medical rescue system has become a top priority.At present,most of the researches related to emergency rescue in China are based on the comprehensive emergency rescue case analysis or experience summary of a certain event(accident)such as natural disasters,safety production,traffic,etc.,while the special research on the emergency medical rescue system is not more common.Especially at the provincial level,it is rare to carry out systematic analysis and countermeasure research on the relevant components of emergency medical rescue.This paper selects two major emergency cases in Henan Province as the starting point,and conducts retrospective case analysis on emergency medical rescue processes such as emergency preparedness,information reporting and response,and centralized treatment,aiming to use the theoretical framework of health systems.Based on this study,the key factors or measures that may affect the rescue effect are explored from the perspective of systematics,and combined with the existing work practice experience,relevant policy recommendations for provincial health administrative departments to optimize the emergency medical rescue system are put forward.Methods:The first is the literature research method:mainly inquire about the important achievements or the latest progress in the theoretical research related to emergency medical rescue in recent years,in order to strengthen the theoretical basis and practical basis of the subject research.The second is the case study method:to select two relatively consistent cases(such as the number of critically injured people and similar event types.)from the major emergencies that have occurred in Henan Province in recent years,and issue and collect them to the participating medical and health institutions.Questionnaire,at the same time,retrieve and consult information reports or work records related to incident disposal,retrospectively sort out incident disposal in chronological order,compare the similarities and differences between the two incident disposals,find out the factors that affect the rescue effects of different cases,and then Put forward countermeasures from the management level.The third is the system analysis method:this project is based on the theory of health system,and takes the whole process of emergency medical rescue as a whole system.Comprehensively analyze the output effects of different systems produced by the key elements of each subsystem input in the rescue system,find out the feasible plan for optimizing the emergency medical rescue system in emergencies,and provide a more feasible methodology for the formulation of policies and measures related to emergency medical rescue in accordance with.Results:1.In the stage of information reporting and emergency response,there is no significant difference between the Changyuan accident and the Xixia accident.2.In the stage of on-site work and centralized treatment,although the transfer and treatment of critically ill patients in the Xixia accident is difficult to transport and treat in large numbers,it is significantly better than Changyuan in terms of the success rate of rescue of critically ill patients within 24 hours(84.6%)Accidents(65.2%).3.In the summary evaluation stage,Nanyang City is superior to Xinxiang City in terms of accident rescue work summary evaluation and application of results.Conclusions:1.Adhere to the standardized management of emergency medical rescue plans,teams,and drills,which can effectively improve the emergency preparedness of emergency medical rescue.2.Carrying out standardized management of emergency information reporting and maintaining the smooth flow of information throughout the emergency medical rescue process is of great significance for the rapid and safe transfer of emergency casualties and the overall planning of rescue forces.3.Concentrating superior treatment forces at the national,provincial and municipal levels,and mobilizing all advanced equipment,advanced technology,and close integration of specialties and other disciplines can play a vital role in improving the success rate of treatment of patients with complex injuries. |
