The Application Of Titanium Mesh(TiLoop Bra)combined With Serratus Anterior Fat Fascia Flap In Immediate Prosthetic Breast Reconstruction For Early Breast Cancer

Objective: To investigate the application of titanium mesh(Ti Loop Bra)combined with serratus anterior fat fascia flap in immediate prosthetic breast reconstruction for early breast cancer.Methods: The clinical data of 61 patients with early breast cancer who underwent prosthesis breast reconstruction after nipple sparing mastectomy(NSM),from January2013 to January 2022 in the Affiliated Changzhou No.2 People’s Hospital were retrospectively analyzed.The patients were divided into 3 groups according to the application of Serratus Anterior Fascia Flap and Ti Loop Bra in reconstruction: titanium mesh group(22 cases of using Ti Loop Bra),fascia group(18 cases of using anterior serratus fat fascia flap)and combined group(21 cases of using Ti Loop Bra combined with anterior serratus fat fascia flap).The operation time,postoperative pain score,postoperative pain duration,average drainage tube placement time,incidence of complications rate,postoperative appearance and patient BREAST-Q questionnaire were compared among the three groups.Results: There were no significant differences in operation time,postoperative pain score and incidence of postoperative complications rate among titanium mesh group,fascia group and combined group(P>0.05).There was no significant difference in postoperative pain score after 24 h among the 3 groups(P>0.05),but the postoperative pain duration in the combined group and fascia group was lower than that in the titanium mesh group(P<0.05).The average drainage tube placement time in titanium mesh group was longer than that in combined group and fascia group,the difference was statistically significant(P<0.05).The one-year follow-up analysis showed that the postoperative appearance score of the titanium mesh group and the combined group was better than that of the fascia group(P<0.05).According to the sore of the Breast-Q questionnaire related to Breast reconstruction,patients in the 3 groups had higher scores on the satisfaction level of breast surgeons,nurses and other doctors,but there was no statistical difference(P>0.05),while the combined group and the titanium mesh group had higher scores on the satisfaction level of breast than the fascia group,with statistical significance(P<0.05).Conclusion: Ti Loop Bra combined with serrate anterior fascia flap prosthesis implantation breast reconstruction after NSM is safe and reliable for early breast cancer patients.The combined group was superior to the titanium mesh group in drainage tube placement time and postoperative pain duration.It was better than fascia group in postoperative appearance and patient BREAST-Q questionnaire score.Compared with the other two groups,there was no increase in operation time,postoperative pain score and incidence of complications rate.It’s a procedure worth promoting.
