Application Of The Autologous Free Dermal Fat Graft In Breast Reconstruction

Objective:Breast cancer is one of the most common types of cancer which threatens women’s health.According to the latest statistics,,there were about four million new cases of cancer in china until 2015.Among these,breast cancer is particularly common in women,which accounted for 15% of the women,with a higher incidence,diagnosis and treatment level of breast cancer corresponding to the rising.Therefore,as the improvement of treatment level,people are not only concerned about survival,but also pay attention to the quality of life after surgery.At this stage,srugery is the main treatment of breast cancer.The patient will cause breast deformation and loss,which may lead to the loss of its unique characteristics of the appearance of women,then they may appear restless mood,anxiety,self-confidence and even autism disorders such as depression after surgery.Thus there have been a lot of breast reconstruction surgery to help improve the quality of life,with the community’s concerning about the living standards of women with breast cancer.The purpose of this study was to explore the clinical value of the free abdominal fat flap for breast reconstruction in clinical practice.Methods:The study collected a total of 8 cases,who using abdominal free dermal fat graft for breast reconstruction patients from July 2015 to June 2016,all patients were female,aging from 25 to 60 years old,6 cases of them were of breast cancer,the other2 cases were benign breast tumor.ALL of the patients were evaluated by conventional color Doppler ultrasound,mammography and breast enhancement MRI.For patients with breast conserving conditions,DCE-MRI based breast computer aided diagnosis(CAD)technique was used to predict the negative margins before surgery,and a thick needle biopsy was performed to determine the nature of the tumor.Complete the above preoperative assessment,patients who would operate,should follow these basicoperation steps : 1.preoperative measurement and implementation design;2.tumor resection and sentinel lymph node biopsy and axillary lymph node dissectionr;3.harvest abdominal fat flap free(autologous free dermal fat graft,FDFG)+ remove the epidermis;4.trimme the free dermal fat flap;5.fill abdominal fat flap free to the breast defect and fix and suture;6.suture abdominal incision.To analyzie the effect of the application of free abdominal fat free flap for the reconstruction of breast in clinical practice by observing the postoperative complications,the patient’s perception of the surgery ‘s results and postoperative follow-up.Results:The application of this method is as follows: 1.the patients who has small breast(especially Asian women).2.the patient who has a wide range of breast resectionof due to various reasons.3.the patient who has tumor in the inner quadrant and central.4.the patient who with a wide range of intraductal papilloma.The incision healed well,the positive rate of the incisal margin,the local recurrence and the survival rate were not affected and the incidence of complications and recurrence rate did not increase after the reconstruction.This procedure can not only achieve the purpose of treatment,but also make the postoperative breast shape didn’t change a lot,bilateral breasts can be basically symmetrical,and achieve the desired results,enhance the patient’s self-confidence,improve the quality of life of patients.It is a kind of operation method with less complication and less time consuming.Conclusion: Abdominal fat free flap for breast reconstruction has so many advantages.For example,it has adequate abdominal tissue,and has a nice cosmetic effect,in addition,The location of the nipple and breasts could keep good symmetry,and because of the material from the secret location,can be carried out for obese patients with abdominal micro plastic and get satisfactory results,which is more in line with the aesthetic requirements of the public.Because of short follow-up time,and the sample size is not enough,the method of observation and statistics to evaluate the long-term effect of breast cancer cure rate and postoperative need longer time,and more clinical experience.
