Objective: To observe the clinical efficacy of auricular acupuncture combined with time-sharing treatment in improving perimenopausal insomnia, and to explore the effect of traditional Chinese medicine combined with midnight-noon ebb-flow theory on sleep. Methods: In this study,a randomized controlled study method was used to select 72 patients with perimenopausal insomnia(45-55 years old)from 2022.01-2022.12 community clinics. They were randomly divided into an observation group(36 cases)and a control group(36 cases).Using the traditional Chinese medicine inheritance auxiliary platform to cluster analysis of auricular points for the treatment of insomnia, sympathetic, shenmen, endocrine, heart, and kidney are commonly used points for auricular acupuncture. The control group selected this group of auricular points to bury the needle. On this basis, the observation group increased the corresponding time points(lung, triple energizer, liver, gallbladder)according to the time of insomnia, and the treatment lasted for 4 weeks. The Pittsburgh sleep quality scale(PSQI),modified Kupperman score,Estradiol(E2),Luteinizing hormone(LH),and other indicators before and after treatment were statistically analyzed. Results :1.Baseline date: There was no significant difference in age, course of the disease,PSQI score,Kupperman score, estradiol, and luteinizing hormone(P < 0.05).2.PSQI score 2.1 PSQI total score comparison Intra-group comparison: the total PSQI scores of the two groups after treatment were significantly lower than those before treatment(P < 0.01),and there was no significant difference between the total PSQI points group after 4 weeks of treatment and 1 month of follow-up(P > 0.05).Comparison between groups: After 4 weeks of treatment, the total score of PSQI in the observation group was lower than that in the control group, with a statistically significant difference(P < 0.05);After 1 week,2 weeks,3 weeks of treatment, and 1 month of follow-up after treatment, there was no statistically significant difference in the total PSQI score between the observation group and the control group(P > 0.05).2.2 PSQI scores Intra-group comparison: the scores of sleep quality, sleep time, sleep time, sleep efficiency, sleep disorder, and daytime dysfunction after treatment were lower than those before treatment, and the differences were statistically significant(P < 0.01).Comparison between groups: the daytime dysfunction score after 1 week and 2 weeks of treatment was lower in the observation group than in the control group, and the difference was statistically significant(P < 0.05);after 3 weeks of treatment, the sleep efficiency score of the observation group was lower than that of the control group, and the difference was significant(P < 0.01).After 4 weeks of treatment and 1 month after treatment, the sleep quality score of the observation group was lower than that of the control group, and the difference was statistically significant(P < 0.05).There was no significant difference in the scores of other indicators(P > 0.05).3.Modified Kupperman score: There was a significant difference before and after treatment in the group(P < 0.01).There was no significant difference between the two groups after treatment(P > 0.05).4.E2 and LH: the difference in hematological level between the follicular phase and the menopausal period after treatment was statistically significant(P < 0.05);there were significant differences in LH after ovulation treatment in the observation group,E2 after ovulation treatment in the observation group,LH after ovulation treatment in the control group, and E2 in the luteal phase after treatment in the observation group(P < 0.05).There was no significant difference in hematological levels between ovulation and luteal phases(P > 0.05).5.Total clinical efficacy: The total effective rate was 90.63% in the observation group and 80.00% in the control group, although the total effective rate in the observation group was higher than that in the control group(P > 0.05),and the clinical effectiveness of 11 cases in the observation group was higher than that in the control group 3 cases, and the difference was statistically significant(P < 0.05).6.Safety: No serious adverse reactions occurred in the two groups during the treatment. Conclusion :1.Auricular acupuncture combined with time-sharing treatment and auricular acupuncture alone can improve the quality of perimenopausal sleep, and the former is better than the latter.2.Auricular acupuncture combined with time-sharing treatment and auricular acupuncture alone can reduce perimenopausal symptoms.3.Auricular acupuncture combined with time-sharing treatment and auricular acupuncture alone can increase serum estradiol levels and reduce serum luteinizing hormone levels.4.Auricular acupuncture combined with time-sharing treatment and a single application of auricular acupuncture have good safety.
