Clinical Study Of Tonifying Kidney And Filling Pulp In Patients With Post-stroke Depression And Mild Cognitive Impairment

Objective To observe the clinical effect of Tonifying Kidney and Filling Pulp in patients with post-stroke depression and mild cognitive impairment,and to explore the efficacy and safety of Shengdi Fujian Prescription in the treatment of Yin deficiency and blood stasis type of patients with post-stroke depression and mild cognitive impairment.Methods A total of 64 stroke patients who met the diagnosis in the outpatient and inpatient department of the Rehabilitation Hospital of the Second Affiliated Hospital of Shandong University of Traditional Chinese Medicine from June 2021 to December 2022 were included in this study.Patients to be enrolled in the study were randomly assigned to treatment or control groups using a random number table.Both groups received routine basic medical treatment and standardized rehabilitation training(including physical rehabilitation training,psychological therapy and cognitive rehabilitation training,etc.).Cognitive rehabilitation training includes computer aided cognitive training,visual feedback training and so on.The treatment group was given Shengdi Fujian Prescription on the basis of the control group.Hamilton Depression scale(HAMD-17),Montreal Cognitive Assessment scale(MoCA),Mini-mental State Examination(MMSE),Fugl-Meyer Motor Function scale(Fugl-Meyer),Stroke Quality of Life scale(SS-QOL),TCM symptom quantification scale and serum BDNF level were analyzed statistically by SPSS 25.0 software.Results A total of 64 qualified cases were collected,32 cases in control group and 32 cases in treatment group.After treatment and before treatment,the two groups of observation indicators were improved in different degrees.There were no significant differences in HAMD-17,MoCA,MMSE,Fugl-Meyer,SS-QOL,TCM symptom quantification scale score and serum BDNF level before treatment(P > 0.05).After 4 weeks of treatment,HAMD-17 and TCM symptom quantification scale scores were lower than before treatment,while MoCA,MMSE,Fugl-Meyer,SS-QOL scale scores and serum BDNF levels were higher in both groups than before treatment,and the treatment group was better than the control group,the difference was statistically significant(P < 0.05).The statistical results of the scores of various factors in HAMD-17 scale showed that there was a statistically significant difference in the scores of depression,guilt,agitation and general symptoms after intervention(P <0.05);The statistical results of each factor score of MoCA scale showed that there was statistically significant difference between the statistical results of attention,abstract and delayed recall scores after treatment(P <0.05).After 2 weeks of followup,HAMD-17 and TCM symptom quantification scale scores were lower in both groups than before treatment,while MoCA,MMSE,Fugl-Meyer and SS-QOL scale scores were higher than before treatment,and the treatment group was better than the control group,the difference was statistically significant(P < 0.05).There were no adverse drug reactions and obvious side effects during the study period.Conclusion Shengdi Fujian Prescription can significantly improve the depression and cognitive ability of patients with post-stroke depression and mild cognitive impairment,enhance the physical activity ability,improve the quality of life,improve serum BDNF level,no obvious adverse reactions were found,and the patient compliance is strong,long-term efficacy lasting.
