| Objective: Through the observation and study of clinical cases,this paper analyzes and discusses the clinical effect of two fixation methods of internal fixation with steel plate and screw and external fixation frame combined with fumigation and washing of traditional Chinese medicine in the treatment of children’s middle and lower tibial fractures,discusses the advantages and disadvantages of the two fixation methods,and then provides methods and ideas for the treatment of children’s tibial fractures.Methods: A total of 50 cases of middle and lower tibial fractures admitted to the Department of Pediatric Orthopedics,Affiliated Hospital of Shandong University of Traditional Chinese Medicine from June 2019 to June 2022 were retrospectively analyzed.According to the different surgical fixation methods,25 cases were divided into plate screw group and 25 cases were external fixator group.Both groups of patients were treated with fumigation and washing of traditional Chinese medicine and functional exercise during the recovery period after operation.Observe and record the general situation of the two groups of cases,the length of operation,the length of hospital stay after operation,the amount of intraoperative bleeding,postoperative complications,clinical healing time of fracture,bone healing time of fracture,functional efficacy evaluation and other data for statistical analysis.To explore the efficacy of two fixation methods and the effect of fumigation and washing with traditional Chinese medicine.Results: All cases in this study were effectively followed up.There was no significant difference between the two groups in terms of age,sex,fracture type,injury mechanism,etc.(P>0.05).There were significant differences between the external fixator group and the steel plate and screw group in terms of operation duration,intraoperative bleeding,postoperative hospital stay and fracture healing time(P<0.05).There was no significant difference in postoperative complication rate between the two groups(P>0.05).In the external fixator group,there were 2 cases of needle passage exudation,2 cases of clip or pin loosening,2 cases of incision fat liquefaction and 1 case of incision line reaction in the plate screw group.Johner-Wruhs tibial fracture efficacy evaluation standard was used to analyze the postoperative efficacy.There was no significant difference between the two groups in the excellent and good rate of efficacy evaluation(P>0.05).Conclusion: 1.Both internal fixation with steel plate and screw and external fixation with external fixator can achieve satisfactory clinical results in the treatment of middle and lower tibial fractures in children,and there is no significant difference in the rate of postoperative complications and the results of the evaluation criteria of the affected limb.The external fixator group has shorter hospital stay,shorter operation rectangle,less intraoperative bleeding,and shorter postoperative bone healing and clinical healing time,that is,the external fixator group has shorter rehabilitation cycle and longer disease duration than the plate screw group.2.In this study,the external fixator group includes a combined external fixator and a single arm external fixator.The nail rod combination is flexible and diverse,and it is easy to adjust during and after surgery,which has a wider applicability.Some patients can perform closed reduction or limited open reduction with the help of C-type arm positioning.The surgical incision is more limited,and the removal of the external fixator does not require secondary surgery,which can reduce the pain of patients while reducing the overall trauma.3.The two groups of patients were externally applied with blood activating and pain relieving fumigant after operation,which can effectively improve the local blood circulation of limbs,relieve swelling,pain,muscle stiffness and other symptoms,promote blood vessel dredging and muscle and bone healing,and improve the effect of rehabilitation training.The clinical application is safe and reliable,reflecting the therapeutic advantages of combining traditional Chinese and western medicine. |
