Professor Li Xiurong’s Medication Rules And Clinical Experience In The Treatment Of Esophageal Cancer

Objective: To summarize the data results based on Professor ***’s herbal prescriptions for the treatment of esophageal cancer,to conduct data mining,to analyze and summarize Professor Li Xiurong’s overall medication patterns and ideas of identification and typing in the treatment of esophageal cancer,and to further explore the tutor’s experience in the clinical treatment of esophageal cancer by combining Chinese and Western medicine,so as to provide reference for the treatment of esophageal cancer.Methods: The data of esophageal cancer cases treated by the tutor were collected and summarized into a table to derive high-frequency drugs and their classification,nature and taste;IBM SPSS Modeler 18.0 was used to conduct association rule analysis to derive commonly used drug pairs;IBM SPSS Statistics 26.0 was used to conduct cluster analysis to derive commonly used drug groups and analyze and compare the patterns of drug selection and dispensing by the tutor.According to the different treatment modalities of esophageal cancer patients,the groups were grouped and Gephi was used to carry out complex network analysis to compare the differences in the instructor’s identification and use of drugs under different treatment modalities,and finally summarize the instructor’s method of drug compounding and ideas of identification and treatment in the treatment of esophageal cancer.Results: A total of 101 cases were collected,with a total of 454 prescriptions involving 227 Chinese herbal medicines.After collating the statistics,combined with the data mining results,it was found that:(1)Esophageal cancer is commonly found in men aged 60-80,with squamous carcinoma being the most common,tumour site mostly in the lower thoracic segment,clinical stage mainly at stage IV,and treatment involving surgery,radiotherapy.Treatment involves surgery,radiotherapy,chemotherapy,targeted therapy and immunotherapy.(2)Analysis of the Chinese herbal formulae reveals that: there are 19 flavours of high frequency drugs: licorice,hemihsia,astragalus,sand,zhebei mum,chenpi,malt,princely ginseng,liushenqu,hawthorn,yujin,maitong,houpu,baijiao,hen’s inner gold,whirling daisy,coix seed,white peony,and wu zhushui.All the medicines have19 types of effects,mainly tonifying the qi,nourishing the yin,clearing heat and detoxifying the toxins,and relieving coughs and phlegm to calm asthma.The drugs are both cold and warm,with bitter,sweet and pungent tastes.The drugs are mainly classified as belonging to the spleen,lung,stomach and liver meridians.(3)The 6 groups of commonly used herbal pairs:Sharen-Sanxian-Hanxia,Hanxia-Chenpi-Zhebei Mu,Sharen-Zhebei Mu,Wu Zhuyu-Huanglian,Hanxia-Huanglian,and Hanxia-Yujin.(4)The 5 groups of commonly used medicine pairs are divided into the following groups according to the combination and efficacy of the drugs: the group of strengthening the spleen,resolving phlegm and dispersing knots,the group of nourishing the lung and benefiting the stomach,the group of clearing the liver and stomach,the group of clearing heat and detoxifying the internal organs,and the group of nourishing blood and calming the mind.(5)Core drugs under the 4 treatment modalities for esophageal cancer:(1)Surgery: Astragalus,Radix Paeoniae Alba,Atractylodes Macrocephalae,Rhizoma Atractylodis Macrocephalae,Radix et Rhizoma Macrocephalae,Radix et Rhizoma Glycyrrhiza Uralensis,Radix et Rhizoma Macrocephalae,Radix prunus ginseng,Radix Chonglou,Poria and Pericarpium Chen.Treatment is based on nourishing the spleen and stomach,promoting the flow of Qi and nourishing blood,and fighting cancer and dispersing nodules.(2)Radiotherapy: Gynostemma lactuca,Astragalus membranaceus,Astragalus membranaceus,Radix et Rhizoma chenopodium,Radix et Rhizoma ginseng,Radix et Rhizoma forsythiae,Radix et Rhizoma scutellariae,Radix et Rhizoma prince’s ginseng,Radix et Rhizoma pentaphyllum,Radix et Rhizoma licorice,Radix et Rhizoma lily.Treatment is based on benefiting Qi,nourishing Yin,clearing heat and moving Qi to move the spleen to resolve phlegm.Treatment is based on benefiting Qi,nourishing Blood and activating Blood circulation,moving Qi,removing phlegm and eliminating stasis.(4)Targeted and/or immunotherapy: Yu Jin,Radix et Rhizoma,Radix Angelicae Sinensis,Radix et Rhizoma Zizyphus,Radix Platycodon,Poria,Atractylodes Macrocephalae,Semen Coix,Radix et Rhizoma Zizyphus,Radix et Rhizoma Glycyrrhizae,Radix Astragali,Radix Codonopsis,Radix Chonglou and Pericarpium Citri Reticulatae.The treatment is based on warming the middle of the body,tonifying the deficiency,opening up stasis and resolving phlegm.Conclusion: According to Professor Li Xiurong,the pathogenesis of oesophageal cancer is a combination of deficiency,qi,phlegm,stasis and toxicity.Treatment is based on nourishing qi and yin and protecting the middle jiao,while different treatments are used to move qi and disperse knots,clear heat and detoxify toxins,dispel dampness and phlegm,and invigorate blood and eliminate stasis to break down cancer toxins.The selection of drugs is refined,the combination of drugs is emphasized,the holistic concept is emphasized,both Chinese and Western medicine are given equal emphasis,the advantages of both are given full play,and the treatment is differentiated according to the characteristics of the disease mechanism at different stages to maximise clinical efficacy.
