Objective:1.Investigate the knowledge,attitude and practice level of premature infants’ position management by NICU nurses and the training needs of premature infants’ position management by NICU nurses.2.Based on the ADDIE model,construct the position management training program for premature infants in NICU nurses.3.Apply the established training scheme to conduct training on position management of premature infants for NICU nurses,and evaluate the training effect and nurses’ training satisfaction.Methods:1.In the investigation and analysis stage,guided by the theoretical model of knowledge,attitude and practice,and using methods such as literature review,research group discussion and expert consultation,the “knowledge,attitude and practice Questionnaire on the Position Management for premature Infants by NICU Nurses” and“Questionnaire on the Training Needs of premature Infants’ Position Management by NICU Nurses” were prepared.NICU nurses from 6 Grade A hospitals in 5 cities of Shandong Province were selected by convenience sampling method.2.In the training program construction stage,the position management training program for premature infants of NICU nurses was preliminarily designed with the guidance of ADDIE model and the previous research results in reference to the methods of literature review and research group discussion;Sixteen experts in related fields were invited as correspondence experts to conduct two rounds of Delphi expert correspondence consultation,and establish a training program of position management for premature infants in NICU nurses based on ADDIE model.3.In the stage of training program application and effect evaluation,the self-before-before-control experimental research method was adopted to select NICU nurses in two districts of a third-class A hospital in Jinan,Shandong Province as training objects to carry out position management training for premature infants.Before and one month after the training,NICU nurses’ knowledge,attitude and practice of Position Management for premature infants were evaluated by using the “knowledge,attitude and practice Questionnaire on the Position Management for premature Infants by NICU Nurses”.After the training,the self-designed “Training Satisfaction Questionnaire” was used to investigate the satisfaction of the NICU nurses participating in the training.Results:1.Survey results of knowledge,attitude and practice and training needs of NICU nurses in position management of premature infants: A total of 201 valid questionnaires were collected.The total score of knowledge,attitude and practice,knowledge dimension,attitude dimension and practice dimension were(147.14±15.15)points,(59.12±7.82)points,(33.10±3.09)points and(84.49±11.32)points,respectively.The standard scores were(86.55±8.91),(84.46±11.18),(94.57±8.83)and(84.49±11.32),respectively.The total score of knowledge,attitude and practice and the score of attitude dimension are at a high level,and the score of knowledge and practice dimension is at a medium level.Multiple linear regression analysis showed that factors such as the nature of hospital,whether they had received relevant training,and the importance of attending relevant training had an impact on the knowledge,attitude and practice scores of premature infants in NICU nurses(P < 0.05).Training demand survey showed that 78.11% of NICU nurses had received training in position management of premature infants;98.01% of NICU nurses were willing to receive position management training for premature infants;91.04% of NICU nurses believed that it was very important to participate in position management training for premature infants;The most desirable training method for NICU nurses is operation demonstration method;The most desirable training methods from high to low are offline teaching,online live broadcasting and online recording and broadcasting.The most desirable training content is the lack of relevant knowledge.The most important obstacle to the postures management of premature infants is the lack of relevant knowledge.The most desirable training content is the influence of different postures on premature infants.2.Construction of training program: After literature review,research group discussion and 2 rounds of Delphi expert letter consultation,the ADDIE model-based NICU nurse position management training program was constructed,including 5 training topics,20 training objectives,20 training contents,training methods including classroom teaching method,operation demonstration method,case seminar method,online course learning method,a total of 8 hours.3.Training program effect evaluation and training satisfaction results: 46 valid questionnaires were collected before and after the training;Before the training,the total score of knowledge,attitude and practice in position management and the scores of knowledge,attitude and practice were(144.85±10.56)points,(54.13±6.04)points,(32.74±2.53)points and(57.98±4.10)points,respectively.After training,the scores were(159.09±11.76)points,(65.63±5.14)points,(33.76±2.45)points and(59.70±5.29)points,respectively.After training,the total score of knowledge,attitude and practice and the score of knowledge and practice were significantly higher than those before training(P <0.05);NICU nurses were 100%,97.8%,100%,97.8%,95.7%,97.8% and 97.8% satisfied with course content,training method,training period,training difficulty,teaching quality of training personnel,significance of practice guidance and overall evaluation,which showed high satisfaction with training.Conclusion:1.NICU nurses have a high level of knowledge,attitude and practice in position management of premature infants and a good attitude,but their knowledge level needs to be improved and their clinical behavior needs to be standardized.NICU nurses have a high demand for position management training of premature infants.2.After two rounds of Delphi expert letter consultation,the ADDIE model based training program of NICU nurses’ position management for premature infants is scientific and practical.3.The ADDIE model based training program of NICU nurses’ position management for premature infants can effectively improve the knowledge,attitude and practice level of NICU nurses’ position management for premature infants,and nurses’ feedback on the training is good,which can be promoted and applied in clinical nursing education. |
