Objective:1.To understand the level of knowledge and practice of narrative nursing of clinical nurses and the current situation of their demand for narrative nursing training;2.Construct and implement the narrative nursing training program based on ADDIE model, analyze the effect of applying the program to improve the narrative nursing ability of clinical nurses, and provide reference for similar research. Methods:This study was designed as a quasi-experimental study with two phases: baseline survey and intervention study.1.Baseline survey:A questionnaire of “Knowledge-Attitude-Practice Survey of Clinical Nurses on Narrative Nursing”compiled by Huang Hui et al.To investigate the current situation of narrative nursing knowledge, attitude and behavior of clinical first-line nurses in each department of a tertiary general hospital in Nanchang.2.Intervention research:(1)Based on the results of previous investigation and relevant theoretical basis, the narrative nursing training program based on ADDIE model was initially proposed, and the narrative nursing training program was modified and improved through expert consultation to determine the narrative nursing training program.(2)Purposive sampling method was adopted to select nurses in two departments with a relatively long space distance and similar level of narrative nursing knowledge and practice as research objects, and they were randomly divided into the intervention group and the control group by coin toss. The control group received regular inhospital training, while the intervention group implemented the training program based on ADDIE teaching model on the basis of regular in-hospital training.(3)Before and after the training, the general data questionnaire, narrative competence scale and the nursing caring characters assessment tool were used to evaluate the narrative nursing ability and humanistic care quality of clinical nurses.(4)After the training, the nurses in the intervention group were asked to fill in the feedback questionnaire to understand the gains, feelings, evaluation and suggestions of the nurses participating in the narrative nursing training course.Results1.Investigation results of narrative nursing knowledge attitude behavior of clinical nurses: 65.65% of clinical nurses have never heard narrative nursing,30.91%of clinical nurses have heard narrative nursing but are not familiar with it,and only1.91% and 1.53% of clinical nurses are familiar or very familiar with narrative nursing. The main ways for nurses to understand narrative nursing are network, books and journals.2.Univariate analysis showed that the scores of narrative nursing knowledge of clinical nurses with different positions were statistically significant(P<0.05);the scores of narrative nursing attitude of clinical nurses with different educational background and professional title were statistically significant(P<0.05);the scores of narrative nursing behavior of clinical nurses with different educational background and positions were statistically significant(P<0.05).Multiplied factor analysis showed that: the position was the relevant factor affecting the narrative nursing knowledge of clinical nurses(P<0.05).Educational background was the relevant factor influencing the narrative nursing attitude of clinical nurses(P<0.05).Position and educational background were the relevant factors affecting clinical nurses' narrative nursing behavior(P<0.05).3.The results showed that 42.98% of clinical nurses thought that narrative nursing could alleviate the contradiction between nurses and patients and promote the communication between nurses and patients;54.11% of clinical nurses suggested to carry out systematic narrative nursing training.4.Comparison between the two groups after training: compared with the control group, the total score of medical narrative ability and the scores of all dimensions in the intervention group were statistically significant(P<0.05);in addition to the humanistic care ability, the total score of humanistic care quality and other scores in the intervention group were statistically significant(P<0.05).5.Internal comparison between the two groups after training:(1)Comparisonbetween before and after training in the intervention group: there was statistical significance in the total score of medical narrative ability and the scores of all dimensions(P<0.05);except for the differences in the scores of humanistic care knowledge before and after testing(P>0.05),there was statistical significance in the total score of humanistic care quality of nurses and the scores of all dimensions(P<0.05);(2)Comparison before and after training in the control group: except for the statistically significant difference in the scores of attention and listening before and after the measurement(P<0.05),there was no statistically significant difference in the total score of medical narrative ability and scores of each dimension(P>0.05).Except that there was no statistically significant difference in the scores before and after the humanistic care concept dimension and the humanistic care perception dimension(P>0.05),there were statistically significant differences in the scores of the total humanistic care quality and other dimensions(P<0.05).6.The evaluation results of the training program and the results of the feedback questionnaire showed that the clinical nurses who participated in the training rated the narrative nursing training path based on ADDIE model highly, and their questions and suggestions were helpful to the subsequent improvement and improvement of the narrative nursing training program.Conclusions1.Clinical nurses have a low awareness of narrative nursing, and the main ways to understand narrative nursing are network, books and periodicals.2.Clinical nurses have a positive narrative nursing attitude, but their narrative nursing knowledge and narrative nursing behaviour are low.Position, educational background and professional title can affect clinical nurses' narrative nursing knowledge-attitude-behaviour level.3.According to the results of baseline survey, the "target point" of training intervention was determined, and a narrative nursing training program based on ADDIE model was constructed: the training objective was to improve the narrative nursing ability of nurses, the group cooperative learning was the training organization form, and the feedback evaluation of each link was emphasized,to achieve the training objective of improving the level of narrative nursing.4.Narrative nursing training for 8 times in this study has a significant effect on improving narrative nursing ability of clinical nurses. The overall level of narrative nursing ability and the scores of attention listening, understanding response and reflection reappearance were higher than those before training, and the understanding response dimension increased the most.5.This training is conducive to the cultivation of humanistic care quality of clinical nurses, and the improvement of humanistic care concept, humanistic care perception and humanistic care ability is relatively high.6.Narrative nursing training program based on ADDIE model provides new ideas for improving narrative nursing ability and humanistic care quality of clinical nurses, which has certain promotion and application value.
