OBJECTIVE:This study further investigated the clinical efficacy of Haitongpi Decoction fumigation on knee osteoarthritis(cold and dampness block)by comparing the Haitongpi Decoction fumigation group with the non-steroidal anti-inflammatory drug Diclofenac Diethylamine Emulgel group,and observing the various indicators before,after and one month after treatment within and between the groups.The observation indicators included VAS score,WOMAC score,Chinese medicine symptom grading scale,and pressure pain threshold at the knee joint.METHODS:Seventy patients with knee osteoarthritis were recruited and randomly divided into a treatment group and a control group,with the treatment group using basic treatment combined with Haitongpi Decoction fumigation,and the control group using basic treatment combined with Diclofenac Diethylamine Emulgel.The treatment period was 3 weeks in both groups,and the efficacy was evaluated before treatment,after treatment and one month after treatment.The efficacy evaluation indexes were mainly VAS score,WOMAC score,Chinese medicine symptom grading scale score and PPT of pressure pain point around the knee joint,and the data were followed up regularly and statistically analyzed.RESULTS:1.There was no statistical difference between the two groups in terms of age,gender,BMI,disease duration,and side of lesion(P>0.05).2.Comparisons within the group:When compared with the pre-treatment period,the VAS scores,WOMAC scores and Chinese medicine symptom gradation scale scores of patients in both groups decreased,and PPT values increased after treatment and one month after treatment,with statistically significant differences(P<0.05).When compared with the post-treatment period,the VAS scores and WOMAC scores of patients in the treatment group increased,and PPT values increased one month after treatment,and the difference was statistically significant(P<0.05),while there was no statistically significant difference in the TCM symptom grading scale scores(P>0.05).When compared with the post-treatment period,the VAS scores,WOMAC scores and TCM symptom grading scale scores of patients in the control group increased and PPT values decreased one month after treatment,and the difference was statistically significant(P<0.05).3.Refer to comparisons between groups,the patients in the treatment group had improved VAS scores,WOMAC scores,TCM symptom grading scale scores and PPT values than the control group at both time points after treatment and one month after treatment(P<0.05).And the difference between VAS scores,WOMAC scores,TCM symptom grading scale scores and PPT values before treatment and one month after treatment were significantly improved in the treatment group than in the control group,the differences were statistically significant(P<0.05).4.Correlation analysis of the efficacy evaluation indexes in the treatment group:By using Pearson and Spearman correlation analysis on the differences of the indexes in the treatment group,the two-by-two correlation analysis of the differences showed that P>0.05,and the correlation coefficients were close to 0,suggesting that there was no linear correlation between the outcome indexes in the treatment group.CONCLUSION:1.Haitongpi Decoction fumigation can effectively relieve the symptoms of knee joint pain,joint coldness,and joint swelling in KOA patients,improve joint function and enhance the quality of life of patients.2.Haitongpi Decoction fumigation has a faster onset of action and better short-term efficacy than topical NSAIDs Diclofenac Diethylamine Emulgel in relieving joint pain,coldness,swelling and joint function.3.No significant side effects were observed during the treatment of KOA by Haitongpi Decoction fumigation,and no abnormal safety indicators were observed in the subjects. |
