Clinical Observation Of Huoxue Zhitong Decoction In The Treatment Of Supination-External Rotation Ⅳ Ankle Fracture Before Operation

Objective:The purpose of this study was to observe the clinical efficacy of adding Huoxue Zhitong decoction to conventional treatment in the treatment of supination-external rotation IV ankle fracture with preoperative swelling and pain,and to provide a basis for clinical promotion and application.Methods:Sixty patients with supination-external rotation IV ankle fracture who were hospitalized in the orthopedic trauma ward of the Affiliated Hospital of Shandong University of Traditional Chinese Medicine between October 2020 and October 2022 were randomly divided into a treatment group and a control group,with 30 cases in each group.The control group was given conventional treatment such as braking,elevation of the affected limb,Sodium Aescinate to reduce swelling,prevention of deep vein thrombosis in the lower limb,and external application of ice nitro powder,etc.The treatment group was given oral blood activation and pain relief soup on the basis of conventional treatment,and the observation process was from the first day of admission to 1 day before surgery.The patient’s affected and healthy ankle circumference,VAS score,and SAS score were recorded before treatment,and the patient’s affected ankle circumference,VAS score,and SAS score were recorded on the 3rd,5th,and 7th days after treatment,and again before surgery.The data were analyzed and processed to compare the VAS score,swelling value,swelling reduction rate,swelling score,preoperative waiting time,SAS score and complication rate related to the treatment process in the two groups.Results:1.By statistical analysis,there was no statistically significant difference between the two groups in terms of general information,pre-treatment VAS score,swelling value,swelling score,and SAS score(P>0.05).2.Intra-group comparison of VAS score,swelling value,swelling score,swelling reduction rate and SAS score of the two groups of patients after systematic treatment achieved significant improvement compared with those before treatment,and the difference was statistically significant(P<0.05).3.When comparing pain scores between groups,VAS scores were significantly lower in the treatment group compared with the control group on days 3,5 and 7,with statistically significant differences(P<0.05).4.The swelling-related indexes were compared between groups.On the third day of treatment,there was no statistical difference in the swelling value,swelling score and swelling reduction rate between the two groups(P>0.05);on the fifth and seventh day,the related swelling observation indexes were significantly improved in the treatment group compared with the control group,and the difference was statistically significant(P<0.05).5.There was no statistically significant comparison of the incidence of preoperative complications between the two groups of patients during treatment(P>0.05).6.The preoperative waiting time in the control group was 7.57 ±1.04 days,which was higher than that in the treatment group of 6.83±0.83 days,and the difference between the two groups of patients was statistically significant(P<0.05).7.There was a statistically significant difference in the preoperative SAS scores between the two groups(P<0.05).Conclusion:The clinical efficacy of Huoxue Zhitong decoction with conventional treatment on the preoperative swelling and pain of supination-external rotation Ⅳ ankle fracture is safe and reliable,which can accelerate the process of swelling reduction,effectively relieve pain,alleviate patients’ anxiety and shorten patients’ preoperative waiting time,and is worthy of clinical promotion.
