Clinical Observation Of Xiaotiaojing Decoction In The Treating Of Early Swelling And Pain After Unstable Ankle Fracture Surgery

Objectives:To observe the effect of Xiaotiaojing decoction on the early postoperative swelling and pain of unstable ankle fracture.In order to provide a new idea and practice basis for the treatment of early postoperative swelling and pain of unstable ankle fracture with traditional Chinese medicine.Methods:According to the diagnostic criteria and selection criteria,60 cases of unstable ankle fracture patients admitted to the department of Microscopic orthopaedics of the affiliated hospital of Shandong university of traditional Chinese medicine were selected.According to the random number table method,the subjects were divided into experimental group and control group with 30 cases each.The experimental group was treated with oral Xiaotiaojing decoction plus or minus combined with "postoperative basic treatment",The control group was treated with " postoperative basic treatment" intervention,Period of treatment among one week.Postoperative basic treatment includes postoperative nursing routine and basic medication,Basic drugs include:Cefotiam hydrochloride for injection,low molecular weight heparin sodium injection,Paracetamo and Dihydrocodeine Tartrate Tablets and Maizhiling tablets.The clinical efficacy observation indexes were VAS score,swelling value,ankle range of motion(flexion and extension)and TCM syndrome score before and after treatment.SPSS19.0 statistical software was used for statistical analysis of all test data.Results:1.Swelling value score:There was no significant difference between the two groups(P>0.05)on the first day(before treatment)and on the third day after operation.There was no significant difference in both two groups on the 3th after operation(P>0.05),and there were significant differences on the 5th and 7th day after operation(P<0.05),When comparing differences before and after treatment.There were significant differences between the two groups on the 5th and 7th day after operation(P<0.05),and the experimental group was superior to the control group.2.VAS score:There was no significant difference between the two groups(P>0.05)on the first day(before treatment)and on the third day after operation.There were significant differences in both two groups before and after treatment(P<0.05).There were significant differences between the two groups on the 5th and 7th day after operation(P<0.05),and the experimental group was superior to the control group.3.Ankle joint mobility:There was no significant difference between the two groups before treatment(P>0.05).There were significant differences in both two groups before and after treatment(P<0.05).There was a statistically significant difference between the two groups after treatment(P<0.05),and the experimental group was superior to the control group.4.TCM syndrome points:The total effective rate was 90%in the experimental group and 60%in the control group.The clinical effective rate of the experimental group was significantly higher than that of the control group(P<0.05).There was no significant difference between the two groups in the total scores of TCM syndromes before treatment(P>0.05),and there was statistical difference after treatment(P<0.05).There was no significant difference in each Symptom between the two groups before treatment(P>0.05).There was a statistically significant difference between the two groups in the group of swelling,pain,ecchymosis and constipation(P<0.05).There was no significant difference between the two groups in the thirst and urinary redness syndrome groups(P>0.05).Except for constipation Symptom in the Control group(P>0.05),all other Symptoms in both two groups are not statistically significant(P<0.05).Conclusion:Xiaotiaojing decoction has definite clinical efficacy in treating the early postoperative swelling and pain of unstable ankle fracture,and can promote the recovery of ankle joint activity and function,improve the main symptoms and signs of traditional Chinese medicine,and optimize the postoperative recovery process of patients,which is worthy of further study and promotion.
