| Objectives:To understand the current situation of elderly frailty in Jishou community,and further analyze its influencing factors,so as to provide theoretical reference for the formulation of elderly frailty intervention measures.Methods:From January 2021 to May 2021,a total of 670 elderly people aged 60 and above in two communities of Jishou City were selected by multi-stage cluster random sampling method.Firstly,Zhenxi Street and Aizhai Town were selected from six streets,five towns and one township in Jishou City.Secondly,Shaziao Community was selected from seven communities in Zhenxi Street and one administrative village.Aizhai Community was selected from two communities in Aizhai Town and 17 administrative villages to conduct a questionnaire survey on all the research objects that met the inclusion and exclusion criteria.Self-made general information questionnaire,The Comprehensive Frailty Assesment Instrument(CFAI),Health Promoting Lifestyle-China and 36 Short Form-36 Health Survey(SF-36)were used as assessment tools.To investigate the status and influencing factors of the elderly in Jishou community.SPSS24.0 and Mplus8.3software were used for statistical analysis of the data.The measurement data were analyzed using mean ±standard deviation for descriptive data.The count data were analyzed using frequency and percentage for descriptive data.Independent sample t test,one-way ANOVA or nonparametric test were used for difference analysis of measurement data.Two tests were used for difference analysis of count data.Multiple linear regression or generalized linear model and potential profile were used to analyze the influencing factors of the elderly ’ s weakness.Results:1.The elderly in the community of Jishou City showed moderate weakness : the total average score of the elderly in the community was(2.06±0.26),and the average scores of the physical,psychological,social and environmental weakness dimensions were(1.24±0.49),(2.09±0.45),(2.49±0.26)and(2.14±0.43),respectively.2.Single factor analysis of the frailty of the elderly in Jishou community :(1)Age,living style,nationality,health status,health-promoting lifestyle,marriage,BMI,educational level,dental function,number of medications,duration of illness and other 16 variables were statistically significant in the total score of frailty in the elderly(P < 0.05).(2)The differences in the scores of 10 variables,such as age,BMI,duration of illness,number of medications,health status,and tooth function,were statistically significant(P < 0.05).(3)Age,occupation,education level,nationality,marriage,BMI,health status,health promotion lifestyle,tooth function and other 16 variables in the elderly mental weakness score difference was statistically significant(P < 0.05).(4)Age,living style,marriage,hearing,dental function,medical insurance type of six variables in the elderly social vulnerability score difference was statistically significant(P < 0.05).(5)The differences in the scores of 12 variables,such as living style,ethnicity,marriage,health status,and health promotion lifestyle,were statistically significant(P < 0.05).(6)Single factor analysis of the potential profile showed that the differences in the total scores of ten variables of age,type of medical insurance,duration of illness,number of medications,sleep,vision,hearing,tooth condition,health status,and healthy lifestyle were statistically significant(P < 0.05).3.Multi-factor analysis of the elderly in community of Jishou City :(1)Eight variables including health status,living style,ethnicity,marriage,BMI,dental function,medical insurance type,and medication quantity entered the regression equation(P < 0.05),and the total explanation for elderly weakness reached 54.4 %.(2)The six variables of health status,age,hearing,tooth function,number of medications and duration of illness entered the generalized linear model of physiological weakness(P < 0.05).(3)Five variables,including health status,marriage,number of medications,BMI and sleep status,entered the regression equation(P < 0.05),explaining 55.6 % of the elderly mental retardation.(4)The four variables of marriage,living style,age and medical insurance type entered the regression equation(P < 0.05),explaining 11.9 % of the social weakness of the elderly.(5)Health status and tooth function entered the generalized linear model of environmental weakness(P < 0.05).(6)Multivariate analysis of potential profile showed that age,duration of illness and health status had statistically significant differences in the scores of the elderly weak subgroup(P < 0.05).Conclusions:(1)The elderly in the community of Jishou City are at a moderate level of weakness,and the differences in the weakness of the elderly are mainly reflected in physiological and psychological weakness.(2)The influencing factors of the decline of the elderly in the community of Jishou City include health status,living style,marriage,BMI,tooth function,medical insurance type,number of medications,and nationality;other factors were sleep,age,hearing,illness time. |
