Objectives1.To understand the current status of primary caregiver preparedness in stroke and to analyze the impact of general demographic information on caregiving preparedness and feelings of caregiving among primary caregivers in stroke.2.To explore the correlations and pathways of action between attachment style, caregiving experience and caregiver preparedness.Methods1.A convenience sampling method was used to select primary caregivers of stroke patients who were hospitalized in a tertiary care hospital in Yangzhou,Jiangsu Province from July 2021 to December 2021 as the study population, and questionnaires were administered to participants who met the inclusion and exclusion criteria. The questionnaire included the general information questionnaire. The questionnaire was entered using Epidata 3.1,a database was created and SPSS 25.0 was used for statistical analysis.Categorical variables were described using frequencies and percentages.Data from all scales were tested for normality, using means and standard deviations to describe continuous variables if they obeyed a normal distribution, and vice versa, using median and interquartile spacing.A t-test or one-way ANOVA was used to analyse the effect of general information on carer preparedness;a multiple linear regression analysis was used to confirm the factors influencing carer preparedness.2.Participants were given a questionnaire for the path analysis part. The questionnaire included Experience in Close Relationships Scale-Revised,Care Preparedness Scale, and Caregiver Reaction Assessment Scale.SPSS 25.0 and Amos 24.0 were used for statistical analysis.Correlation analysis was conducted to analyse the two-by-two relationship between self-esteem,caregiving burden and attachment dimensions and caregiver preparedness.A path model of caregiver attachment patterns to caregiver readiness was constructed using Amos 24.0,parameter estimation was conducted using the maximum likelihood method, model structure was adjusted for corrections based on the correction index and expertise.Results1.A multiple linear regression analysis was conducted using the mean of the total caregiver preparedness scale as the dependent variable and the demographic variables that were statistically significant in the univariate analysis as the independent variables. The independent and dependent variables all followed a normal distribution. The results showed that caregiver gender, place of residence, monthly household income, and number of co-caregivers had a significant effect on caregiver preparedness, specifically, the higher the gender was male, the place of residence was county and city, the higher the monthly household income, and the higher the number of co-caregivers,the higher the caregiving preparedness.2.60.2%of caregivers were female, and the average age was(55.17± 12.98).Attachment avoidance was negatively associated with self-esteem and caregiver preparedness(r=-0.487,P<0.01;r=-0.322,P<0.01),positively associated with caregiver burden(r=0.357,P<0.01).Attachment anxiety was positively correlated with self-esteem(r=0.221,P<0.01),and negatively correlated with caregiver preparedness(r=-0.194,P<0.01).Taking caregiving self-esteem as the mediating variable, the total effects of attachment anxiety and attachment avoidance on caregiving preparedness were 0.035,P=0.005 and-0.330,P<0.001.Taking caregiving burden as the mediating variable, the total effects of attachment anxiety and attachment avoidance on caregiving preparedness were-0.044,P=0.042 and-0.312,P<0.001.ConclusionsAttachment orientation, particularly avoidant attachment,is an important factor inunderstanding caregiving reactions and preparedness for care in primary caregivers of stroke patients. There were correlations among attachment orientation, caregiving reactions and caregiver preparedness. From the perspective of attachment theory, medical staff can pay attention to caregivers with insecure attachment, give appropriate support, and further promote the physical and mental health of caregivers.
