Objective:To compare the clinical effects of Bone Transport with a External Locking Plate and Ilizarov External Fixator in Tibial Bone DefectsMethods:43 patients with tibial bone defect treated in our hospital from January 2016 to September 2020 were analyzed retrospectively.Among them,23 cases were treated with Ilizarov stent and 20 cases with BTLP system.Three-stage bone transport operation was used for internal fixation in both treatment groups.First,the sex,age,height,weight,cause of injury,complications,fracture classification(Gustilo classification)and other general data were compared between the two groups.Then,the operation time,bone transport time,bone defect length,external fixation index and rack length were recorded in the two groups.Finally,after the end of bone remodeling treatment,Ilizarov Method Applied Research Institute(ASAMI)was used to evaluate the curative effect of bone healing and function,and the postoperative complications of the two groups were counted.Results:The follow-up time ranged from 13 to 47 months(average 22.15 months).There was no significant difference in general data,bone transport time and bone defect length between the two groups(P>0.05);The duration of operation and external fixation index in BTLP group were significantly lower than those in Ilizarov group(P<0.05);BTLP group and Ilizarov group had similar bone healing evaluation(excellent:6 vs 5;Good:12 vs 10;Fair:4 vs 3;Poor:1 vs 2),but there was no significant difference(P>0.05);The prognostic function of BTLP group was higher than that of Ilizarov group(excellent:1 VS 8;Good:9 VS 10;Fair:10 VS 2;Poor:3 VS 0),the difference was statistically significant(P<0.05);Among the postoperative complications,the incidence of nail infection,joint stiffness and pain in BTLP group was lower than that in Ilizarov group,and the difference was statistically significant(P<0.05).Conclusions:BTLP is safe and effective in the treatment of tibial bone defects,and has significant advantages over the traditional Ilizarov technique in terms of operative time,external fixation index,and functional recovery of the affected limb,etc.BTLP deserves to be widely used in clinical practice. |
