Objective: In this paper,the clinical datum of 126 women with polycystic ovary syndrome(PCOS)and 82 women of childbearing age were retrospectively analyzed,to investigate the relationship between the level of different 25(OH)VD in the serum and metabolic characteristics of patients with PCOS,and to observe the influence of the level of 25(OH)VD on the function of ovarian stimulation and pregnancy complications among patients with PCOS.Method: Patients treated in Reproductive Medicine Center of our Hospital from January 2019 to December 2020 were screened.There are 126 PCOS patients diagnosed according to Health Industry Standards of the People’s Republic of China for the Diagnosis of Polycystic Ovary Syndrome formulated by the Ministry of Health in 2011,which complicated with ovulation disorders and treated with ovulation induction were included in the study group,and 82 women of normal reproductive age who underwent pre-pregnancy physical examination in our department during the same period were included in the control group.We retrospectively analyzed their medical records which including serum 25(OH)VD,sex hormones:follicle stimulating hormone(FSH),interstitial cell stimulating hormone(LH),testoterone(T),LH/FSH;four items of blood lipids:total cholesterol(TC),triglyceride(TG),low density lipoprotein(LDL),high density lipoprotein(HDL);fasting plasma glucose(FPG),fasting insulin(FINS),insulin resistance index(HOMA-IR)and other serological index,as well as their age,height,weight,body mass index(BMI)and past medical history and so on.SPSS 26.0 statistical software was used to compare the differences of various indexes between PCOS patients and healthy women.According to different serum 25(OH)VD levels,PCOS patients were further divided into three groups: 25(OH)VD severe deficiency group,25(OH)VD deficiency group and 25(OH)VD normal group.The correlation between serum 25(OH)VD level and various indexesand its influence on ovulation induction effect and pregnancy outcome were analyzed.Results:1.There were no statistically significant differences in age,TC,TG,LDL,HDL between PCOS patients and normal women(P >0.05),while BMI,LH,LH/FSH,T,FPG,FINS,HOMA-IR and other indexes in PCOS group were higher than those in control group(P<0.05).The levels of FSH and 25(OH)VD in this group were lower than those in the control group,and the differences were statistically significant(P<0.05).2.Comparison of different serum 25(OH)VD levels in PCOS patients,There were no significant differences in age,FSH,LH,LH/FSH,TG and FPG among PCOS patients with different 25(OH)VD levels(P>0.05).BMI of the 25(OH)VD severe deficiency group was significantly higher than that of the deficiency group and the normal group,and the difference was statistically significant(P<0.05).The levels of T,FINS and HOMA-IR in 25(OH)VD severe deficiency group were higher than those in normal group and deficiency group,and the differences were statistically significant(P<0.05).The HDL level of 25(OH)VD normal group was higher than that of severe deficiency group and deficiency group,while LDL level was lower than that of deficiency group and severe deficiency group,and the difference was statistically significant(P<0.05).There was no correlation between age,FPG,LH,FSH,LH/FSH,TC,TG,LDL and25(OH)VD(P>0.05),but BMI(r=-0.199),T(r=-0.19),FINS(r=-0.26),HOMA-IR(r=-0.22)and 25(OH)VD level are negatively correlated(P<0.05),and a positive correlation between HDL(r=0.152)and 25(OH)VD level(P<0.05).3.Comparison of ovulation induction results of PCOS patients with different serum25(OH)VD levels showed that the endometrial thickness of the three groups was different in different 25(OH)VD groups on ovulation day,but the difference was not statistically significant(P>0.05),while the cycle ovulation rate of 25(OH)VD severe deficiency group was statistically lower than that in the deficiency group and the normal group,and the difference was statistically significant(P<0.05).4.Compared with pregnancy outcomes of different serum 25(OH)VD levels,the cyclic clinical pregnancy rates of 25(OH)VD severe deficiency group,deficiency group and normal group were 12.3%,13.7% and 38.5%,respectively,the periodic clinical pregnancy rate of 25(OH)VD normal group was higher than that of severe deficiency group and deficiency group(P<0.05),but there were no statistical differences in early abortion rate,premature delivery rate and full term birth rate among the three groups(P>0.05).Conclusion:1.The serum 25(OH)VD levels of PCOS patients was lower than that of healthy women of childbearing age.2.Serum 25(OH)VD levels in PCOS patients were negatively correlated with BMI,HOMA-IR,T and FINS,and positively correlated with HDL.3.Compared with the 25(OH)VD normal group,the periodic ovulation rate of PCOS patients in the severe deficiency group was significantly lower,but there was no significant difference between the deficiency group and the normal group.4.There was no significant difference in the cycle clinical pregnancy rate of PCOS patients between the 25(OH)VD severe deficiency group and the deficiency group,but both groups were significantly lower than the normal group.
