OBJECTIVETo observe changes of clinical parameters and biochemical ingredients in gingival crevicular fluid (GCF) in adolescents with fixed orthodontic appliances and evaluate the effects of the fixed orthodontic appliances to the health of the periodontal tissues.METHODSSubject and Population40 subjects with fixed orthodontic appliances aged between 11 and 18 years were selected for the study. They came from the department of orthodontics in the Stomatology College of China Medical University. None of them had used any antibiotics in the latest 2 months or had any systematic diseases. All of them should have good oral hygiene.Subjects were checked before and at the 1 month, 2 months, 3 months after wearing the fixed orthodontic appliances. The left upper first molar and the right incisor were choosed as indexed teeth.Clinical measurement and samplingThe sites to be sampled were isolated with cotton rolls, supragingival plaque removed and the site gently dried with an air syringe. 30s later GCF was collected with prefabricated paperstrips. The strip was inserted into the pocked until mild resistance was felt and kept there for 30s. Samples were stored at -70 C until analyzed. The weights of strips were measured with the electronic balance. The difference of the weight before and after sampling was recorded asthe volumes of the GCF.Clinical parametersAt the site the plaque index (P1I) , bleeding on probing (BOP) , probing depth ( PD) and the attachment loss ( AL) were recorded.Biochemical ingredients4. 1 All samples were dissolved in 1.5mL PBS buffer(0.01mol/L,pH7.4) for 1 h,centrifuged at 4C in 10,000rpm for 10min.EA activity: To the 100l sample, 100l ImM substrate ( Meosuc- ala-ala- pro- val- NA,sigma)was added. The mixture was incubated at +37C and after two hours the absorbance at 405 nm was measured in a Bio-Tek . The recorded Abs represents the hydrolysis for 2h and presented as arbitrary units per site.MPO activity: ultra spectatorRESULTSChanges in clinical parameters over timeThere were significant increase in PIJ, BOP and PD ( p <0.001) after the subjects wore the fixed orthodontic appliances. None of them have AL.Changes in the biochemical ingredients over timeThere were significant increase in the volume of GCF, the GCF-EA and the GCF-MPO activities after the subjects wore the fixed orthodontic appliances. The changes in the molars were more signiticont than the changes in the upper incisors (p <0. 001).3. Regression analysis showed that the changes in clinical parameters were significant related to the change in the volume of the GCF and the biochemical ingredients in the GCF: GCF-EA,GCF-MPO.CONCLUSION1. There were significant increases in the clinical parameters and the biochemical ingredients in the GCF after wearing the fixed orthodontic appliances.2. The biochemical ingredients were significant related to the clinical pa-rameters.3. The biochemical ingredients analysis was more sensitive than the clinical parameters in monitoring the periodontal health in the orthodontic therapy.
