| Objective:By exploring the clinical efficiency, patient satisfaction and complications of SVF-gel(Stromal Vascular Fraction-gel)and granular fat in improving nasolabial folds to provide reference for clinical application.Methods:50 female patients who need nasolabial groove treatment in Plastic and Cosmetic Surgery Department of Qingdao University Affiliated Hospital from August 2020 to August 2021 were randomly divided into experimental group and control group,with25 patients in each group. The experimental group was injected with SVF-gel and the control group was injected with granular fat.The “nasolabial fold treatment questionnaire” was issued in 1 month,3 months and 6 months respectively(see Appendix 1).The main contents of the questionnaire include: according to the Wrinkle Severity Rating Scale(WSRS),the current score is divided into absent(1point,no visible fold;continuous skin line),mild(2 points, shallow but visible fold with a slight indentation),moderate(3 points, moderately deep folds but not when stretched),severe(4 points, very long and deep folds but less than 2 mm visible fold when stretched)and extreme(5 points, extremely deep and long folds;2–4 mm visible fold when stretched).According to the WSRS,the preoperative WSRS score was set as the baseline, and the decrease of postoperative WSRS score ≥ 1 was defined as effective, and the effective rate was evaluated in 1 month,3 months and 6 months.According to the Patient’s Aesthetic Improvement Scale(PAIS),the evaluation was divided into optimal improvement, good improvement, minimal improvement,no change and worse than before treatment. The satisfaction rate in 1 month,3 months and 6 months after operation was counted, and the satisfaction rate =(obvious improvement cases + better improvement cases)/ total number × 100%.The incidence of postoperative complications was counted.SPSS 26.0 was used for statistical analysis, and the clinical effects of SVF-gel and granular fat were compared and discussedResults:1.Comparison of WSRS score in the group: experimental group: 3.08 ± 0.64 before operation,1.88 ± 0.67 after 1 month,1.96 ± 0.74 after 3 months and 2.28 ±0.79 after 6 months;control group: 2.84 ± 0.75 before operation,1.68 ± 0.63 after 1month,2.04 ± 0.84 after 3 months and 2.32 ± 0.95 after 6 months; The WSRS score of each month after operation in the two groups decreased compared with that before operation, and the difference was statistically significant(P < 0.001).2.Comparison of efficiency between groups: 1 month after operation, the experimental group was 100%,the control group was 96%,the difference was not statistically significant(P > 0.05);3 months after operation, the experimental group was 96%,the control group was 76%,the difference was not statistically significant(P > 0.05);6 months after operation, the experimental group was 80%,the control group was 52%,the difference was statistically significant(P < 0.05).3.Comparison of satisfaction rate between groups: 1 month after operation,t the experimental group was 100%,and the control group was 92%,the difference was not statistically significant(P > 0.05);3 months after operation, the experimental group was 84%,and the control group was 64%,and the difference was not statistically significant(P > 0.05);6 months after operation, the experimental group was 68% and the control group was 40%,the difference was statistically significant(P < 0.05).4.Complications: There were no local induration, infection, necrosis and bilateral asymmetry in both groups.Conclusion:SVF-gel and granular fat as autogenous fat fillers have good effect and high safety in the treatment of nasolabial folds. With the passage of time, the WSRS scores of both SVF-gel and granular fat increased, and the filling effect became worse due to partial absorption of fat.In terms of persistence, the effectiveness of SVF-gel filling is better than granular fat,and patient satisfaction is higher, which has the value of clinical promotion. |
